Objective: To analyze the clinical data of patients with alcoholic cardiomyopathy and dilated cardiomyopathy in the first Affiliated Hospital and the three Affiliated Hospital of Zunyi Medical College, and to understand the clinical characteristics of ACM patients. Methods: Collate the clinical data for the patients with dilated cardiomyopathy and alcoholic cardiomyopathy at department of cardiology in the first Affiliated Hospital and the three Affiliated Hospital of Zunyi Medical College from Jan 2013 to Dec 2015. 40 cases of healthy patients who has a large number of drinking history, and normol cardiac structure and function was control group. Compare the difference of age, history, electrocardiogram, and echocardiography between the DCM group, ACM group and alcohol group patients.Results:1. A total of 89 cases of ACM patients(all men),and 560 cases of DCM patients(341 male, 219 female, The ratio of male to female was 1.6:1) were enrolled.ACM patients accounted for 0.35%(89/25191)in the total hospitalized patients in the Department of cardiology, and DCM patients accounted for 2.22%(560/25191),the ratio of ACM to DCM was 1:6.29. The highest proportion of heart function classification in ACM and DCM group was NYHA III which was 58.4%(52/89),59.5%(333/560) respectively.2. The result of echocardiography showed DCM and ACM group were similar, but the proportion of all cardiac chambers enlargement in ACM group was significantly higher than that in the DCM group(49.4%,44/89). Most DCM patients showed enlargement of left cardiac chambers(69.5%,389/560).ACM patients can appear a variety of arrhythmia, the most common arrhythmia was atrial arrhythmia and left bundle branch block. In DCM group, the left bundle branch block proportion was significantly higher than that of ACM group.3.Compare with the control group, the serum level of UA、Cr,AST,ALT、 NT-pro BNP in ACM group and DCM group were higher. Compare with DCM group, the levels of UA, AST, ALT were significantly higher in ACM group, the levels of HDL-C was decreased significantly and UA was higher significantly than other two groups. Conclusion:During the three years, When the ACM patients come to the hospital, the heart function of most of them have reached NYHA III.In ACM patients, the echocardiography showed the proportion of fully expanded heart was more obvious.the electrocardiogram of ACM patients was often accompanied by a variety of cardiac arrhythmias, which was more common in atrial arrhythmia and left bundle branch block. In ACM patients, the level of HDL-C was significantly decreased, the level of UA concentration increased significantly, which increased with the severity of heart function.
