Objective: In order to solve the above shoulder jiont small splint external fixation in the treatment of proximal humeral fractures with a fixed weak, easy to slide, axillary press-fitting,ect, on the basis of the principle of small splint fixed \ "flexibility\" and \ "action\" theory, to design a kind of fixation, attached with a good, and the economic, safe and convenient to use the anatomical shape of plywood, perfecting applied to clinical, after treatment of proximal humeral fractures in the elderly, compared with the clinical curative effect of surgical internal fixation treatment, evaluation of curative effect, for clinical widely used type anatomical shape of plywood, rich small splint external fixation method for the treatment of proximal humeral fractures.Methods: Design and production of proximal humeral fractures anatomical shape of plywood,with 60 cases of elderly(aged 60 or higher) in patients with proximal humeral fractures as the research object. In gansu university of traditional Chinese medicine hospital orthopaedics,according to the inclusion criteria, collected in January 2014- July 2015 included in the standard case 60 cases of proximal humeral fractures, of which the female 43 cases, 17 were male, aged 60 to 78 years old. According to the principle of random distribution, all patients were randomly divided into two groups according to their time of priorities, the treatment group and control group, 30 cases each. Treatment group with anatomical shape splint external fixation treatment, control group adopts PHILOS beloved or locking plate internal fixation treatment, the treatment cycle is 2 weeks. Followed for 24 weeks, observation records followed for 24 weeks respectively after pain, daily life limited degree, shoulder joint strength, shoulder joint activity, and the fracture, fracture and 4 weeks after treatment of clinical healing time, 12 weeks and 24 weeks after callus growth situation,according to the results of statistical analysis of various data, evaluation of proximal humeral fractures in the elderly type anatomical shape splint treatment curative effect.Results: Treatment group and control group in age, sex, and there is no obvious difference of fractal fracture. According to the Constant Murley score shoulder joint function score standard system, divided into the treatment group judgement(73.08±7.38) points, were divided into control group judgement(65.62±8.80) points, with significant difference(P<0.05). Treatment group in daily life, pain, shoulder joint activity, callus growth, fracture clinical healing time was for the best, the control group in the reduction of fractures of the treatment group. Deltoid muscle power recovery in two groups have no significant difference(P>0.05)Conclusions: Followed for 24 weeks, the anatomy of the shaping splint external fixation treatment of proximal humeral fractures in the elderly in pain relief, daily life ability of recovery,the recovery of shoulder joint mobility, callus growth time, fracture is superior to the clinical healing time locking plate; Locking plate internal fixation for fracture of counterpoint is more advantage, the recovery of muscle strength has no difference between the two treatment methods. In the treatment of proximal humeral fractures in the elderly, type anatomical shape fixation splint external fixation in the treatment, obtain reliable curative effect, and has the shape, attached with, comfort, easy to wear, low cost, better adjustment of the advantages of less worthy of promotion.
