Objective:We screen and analyze the related risk factors of stroke of middle-aged and elderly in Phoenix Health Service,Baota District,Yan’an City,Shanxi Provence,China,In order to understand the distribution and composition of the regional population about the risk factor of stroke,further to provide better advice about stroke prevention activities and the prevention of stroke can be more targeted,so we can achieve positive effect.Methods:Choose the community where is close to the Yan’an University Affiliated Hospital and Phoenix Health Service Baota District works for.After Health Service Center ’ s medical personnel training and testing qualified by specialists from Affiliated Hospital of Yan’an University Cardiovascular Disease Hospital.They will make an appointment with local residents who aged 40 years old and above in the community(who have left over half a year can be excluded) by telephone at service center,to perform questionnaire investigation in a face-to-face way and have physical examination by simple methods,then filling out the sheet 1 from《Statistic table for the pilot work information of screening and intervention in stroke》.After accumulating the scores of risk factors,the high-risk groups that are defined by medical personnel from Heal Service Center will complete the part three of sheet 3:”Stroke screening of high-risk individuals”.Those group will get the carotid artery color Doppler ultrasound(Philips IE33) and biochemical examination without any expenses from Affiliated Hospital of Yan’an University,and theyalso need to make further physical examination according to one’s will..Collecting data from the Sheet and inspection results, SPSS 19.0 software for statistical analysis, the basic data using frequency measurement data analysis. Chi square test are performed to determine the risk factors of stroke.Outcomes:In this survey, 2943 questionnaires were distributed, 2190 valid questionnaires, the effective recovery rate was 74.41%,the average age(63.44 + 11.20) years old, male 977cases(44.61%),female 1213 cases(55.39%).High-risk population of stroke were 375 cases in this survey, Constitution ratio 17.12%,middle-risk population of stroke were 937 cases,Constitution ratio 42.79%,the low-risk population of stroke were 878 cases,Constitution ratio 40.09%.At high risk of stroke in 375 cases,140 cases were male(37.33%),the average age(65.09 + 9.43) years old, female 235 cases(62.67%),the average age(64.43 + 9.19)years;there were 26(9.63%)cases at the age of 40~49 years old;there were 96(16.84%)cases at the age of 50 to 59 years old;there were 135(17.16%)cases at the age of 60 to 69 years old;there were 107(18.23%)cases at the age of 70 to 79 years old;there were 11(7.84%)cases over the age of 80 years old.The large numbers residents’ degree of education was junior high school. The risk factors of stroke’s distribution in high-risk population:281 cases with hypertension,account for 74.93% of the total number of high-risk smokers;82 cases with smoke, account for 21.87% of the total number of high-risk drinkers;21 cases with drink,account for 5.6% of the total number of high-risk groups;167 cases with dyslipidemia, account for 44.53% of the total number of high-risk groups;diabetic patients were 122 cases,accounting for 32.53% of the total number of high-risk groups;overweight(BMI≥26kg/m2) 137 cases,accounting for 36.53% of the total population at high risk group;66 cases with heart disease,account for 17.60% of the total number of high-risk population;family history of stroke were 53 cases,account for 14.13% of the total number of high-risk groups;Lack of physical exercise were 307 cases, account for 81.87% of the total number of high-risk groups;Exceed the Hcy standard were 324,account for 86.40% of the total number ofhigh-risk groups.The comparison of risk factors of stroke in high-risk groups in different gender:In addition to family history of stroke not considered significant difference between male and female,hypertension,smoking,drinking and dyslipidemia,diabetes,overweight(BMI ≥ 26kg/m2),lack of physical exercise,heart disease were considered statistical significance.The distribution of high risk factors in different gender:The distribution of high risk factors in male:lack of physical exercise> hypertension>smoking> dyslipidemia>overweight>diabetes>drinking>family history of stroke>heart disease;The distribution of high risk factors in female:lack of physical exercise>hypertension> dyslipidemia> overweight> diabetes> heart disease> family history of stroke> smoking> drinking(see table).The result of carotid artery color Doppler ultrasound:77 cases(20.53%) with thickened carotid artery intima;211 cases(56.27%)with carotid artery plaque;12 cases(3.20%) with carotid artery stenosis.Comparison of the results of carotid artery ultrasound in different age:different age in groups at high risk of stroke carotid artery intimal thickening, plaque, carotid artery stenosis was statistically significant(P<0.05);40- and 50- years old stroke patients’ difference is no significant;70- and 80-years old stroke patients’ difference is no significant(P>0.005);40- and 50-years old,the rate of carotid artery intimal thickening,plaque,carotid artery stenosis less than 60 years old,70 years old and 80 years old;60 years old,the rate of carotid artery intimal thickening,plaque,carotid artery stenosis less than 70 years old and 80 years old;the differences were statistically significant(P<0.005).Conclusion:60- years old are majority of high-risk residents in screening region.Hypertension and Lack of Physical exercise are the most important risk factors for stroke in community resident.Therefore,we should provide extensive health education,strengthen physical exercise,cultivate good living habits and dietary habits,and strengthen the management of hypertensive patients.The result of artery color Doppler ultrasound indicated:There were large numbers residents have pathological changes of carotid artery at the age of 60 and70 years old.Under the age of 60 is the optional age to prevent the pathological changesof carotid artery.
