Objective:To explore the related risk factors of carotid atherosclerosis and intracranial artery stenosis in population with high risk of stroke in Jiangling community.To provide the basis for the formulation of stroke prevention and control strategy.Methods:A survey was conducted among the population older than 40 years old in Jiangling community.We conducted a questionnaire survey among the sample population,and filled in the <risk assessment table of the high risk population screening and intervention project> formulated by the National Health Planning Commission,then the population with high risk of stroke were screened by the criteria for judging high risk of stroke.We carried out a detailed questionnaire survey of the high-risk groups,and further tested blood glucose(GLU),glycosylated hemoglobin(HbA1c),blood lipids,homocysteine(Hcy),and detected carotid artery and intracranial artery by color doppler ultrasound and transcranial doppler(TCD).We analyzed the basic characteristics of carotid atherosclerosis and intracranial artery stenosis in high-risk population.We performed a correlation analysis between carotid intimal thickening,carotid plaque formation,carotid artery stenosis,intracranial artery stenosis and the related factors.We used the SPSS22.0 to analysis the data,and P<0.05 reflect the correlation was statistically significant.Results:(1)A total of 162 people were included in this study,including a total of 92 men(56.8%)and a total of 70 women(43.2%).The mean age was 65.35±7.25,and the mean age of the male was 65.54±7.66,the mean age of the female was 65.09±6.72.The education level of male was significantly higher than that of female(P<0.05).The exposure rate of hypertension,smoking and Hcy levels were significantly higherin men than in women(P<0.05),but the exposure rate of atrial fibrillation,lack of exercise,and the level of total cholesterol(TCHO)were higher in women than in men(P<0.05).(2)The thickness of carotid artery intima-media thickness(IMT)in male was higher than that in female(P<0.05).There was no significant difference between male and female in carotid plaque,carotid stenosis and intracranial artery stenosis(P<0.05).(3)Carotid IMT is closely related to advanced age,hypertension,smoking,elevated low density lipoprotein cholesterol(LDL-C),decreased high density lipoprotein cholesterol(HDL-C)and elevated Hcy(P<0.05),high blood pressure only affects men(P < 0.05).We made a multiple logistic regression,the results showed age,smoking and HDL-C enter the regression equation.(4)The level of GLU,HbA1 c and Hcy were higher in patients with carotid plaque formation than those without carotid plaque formation(P < 0.05).Among men,the exposure rate of diabetes mellitus,overweight or obesity in patients with carotid plaque formation were higher than those without carotid plaque formation(P<0.05).(5)Elevated Hcy is closely related to carotid artery stenosis(P<0.05).(6)The exposure rate of diabetes mellitus and family history of stroke in patients with intracranial artery stenosis was significantly higher than that in the non-stenosis group(P <0.05),and the GLU and HbA1 c levels were significantly higher than those without stenosis(P < 0.05).We made a multiple logistic regression,family history of stroke and GLU enter the regression equation.Conclusion:(1)Age and sex are risk factors of carotid atherosclerosis in high risk population of stroke of the community.With the increase of age,the carotid IMT increased.carotid IMT in female was thicker than that in female.(2)Older age,hypertension,smoking,elevated Hcy,elevated LDL-C and decreased HDL-C are risk factors for carotid intimal thickening in the high risk population of stroke of Jiangling community.Among them,the elderly,smoking anddecreased HDL-C are independent risk factors.(3)Elevated GLU and elevated Hcy are risk factors for carotid plaque formation in the high risk population of stroke of Jiangling community.(4)Elevated Hcy is an risk factor for carotid artery stenosis in the high risk population of stroke of Jiangling community.(5)Diabetes mellitus,family history of stroke and elevated GLU are risk factors for intracranial artery stenosis.Among them,family history of stroke,elevated GLU are independent risk factors for intracranial artery stenosis.
