Objective: To discuss the value of transrectal strain elastography in diagnosing prostate cancer and benign prostate disease.Methods: 77 patients underwent 10-scores systematic prostate biopsy in our hospital were enrolled in this study. The clinical information of serum prostate-specific antigen(PSA) and digital rectal examination(DRE), and the appearance of magnetic resonance imaging(MRI), transabdominal ultrasound(TAS), transrectal ultrasound(TRUS), transrectal real-time elastography(TRTE) were analyzed and compared with pathological results. The coherence of the strain scores by TRTE with the pathological results of biopsy local areas were analyzed, and the value of transrectal strain elastography in differential diagnosis of prostate cancer and benign prostate disease was evaluatedResults: 1. 32 cases among 77 patients were diagnosed as prostate cancer, 45 cases were diagnosed as benign prostate disease. 141 cases among 462 prostate biopsy local areas were diagnosed as prostate cancer, 321 cases were diagnosed as benign prostate disease. 2. There was significant difference between prostate cancer and benign prostate disease in PSA, DRE, MRI, TAS, TRUS, TRTE(P<0.05). 3. The sensitivity of transrectal strain elastography in diagnosing prostate cancer and benign prostate disease was 81.25%, which was higher than that of TAS(31.25%) and TRUS(65.63%). Specificity was 86.67%, which was higher than that of PSA(26.67%)、DRE(42.22%)、MRI(71.11%)、TRUS(75.56%). Positive predictive value was 81.25%, which was higher than that of PSA(47.62%)、DRE(50.00%)、MRI(69.05%)、TAS(76.92%)、TRUS(65.63%). Consistent rate was 84.42%, which was higher than that of PSA(54.55%)、DRE(58.44%)、MRI(79.22%)、TAS(67.53%)、TRUS(71.43%). 4. The strain scores of TRTE were significant difference between prostate cancer and benign prostate disease in 77 whole prostate and 462 biopsy local areas(P<0.05). 5. The strain scores of TRTE was significant difference between prostate cancer and benign prostate disease in the six systematic prostate biopsy local areas(P<0.05). 6. The 3 points of the strain score was considered asthe cutoff point of standards of prostate cancer and benign prostate disease, because it can conformed with 84.42%(65/77) whole prostate and 74.03%(342/462) systematic prostate biopsy local areas.Conclusion: 1. Transrectal strain elastography has high diagnostic value for the differential diagnosis of prostate cancer and benign prostate disease. 2. The strain scores in whole prostate and biopsy local areas of prostate cancer is higher than that of benign prostate disease. 3. The strain scores in biopsy local areas is also valuable in differential diagnosis of prostate cancer and benign prostate disease as well.
