Objective : To explore the differential diagnosis of transurethral ultrasound shear wave elastography for benign prostatic hyperplasia and prostate cancer.Methods: A total of 120 male patients who underwent transrectal ultrasound and real-time shear wave elastography of prostate in our hospital and underwent 12-needle systematic ultrasound-guided biopsy were selected.Intraoperatively,the area of interest of ultrasound elastography was corresponding to the puncture site.The hyperplasia group was divided into two groups by routine paraffin pathology and the cancer group by routine paraffin pathology combined with immunohistochemical results.Results:212 needles of 80 patients with benign prostatic hyperplasia and 125 needles of 40 prostate cancer patients,The measured glaucoma of the prostate gland corresponded to the site of the transrectal ultrasound guided prostate biopsy for pathology.Transreural ultrasound prostate shear wave elastography,three elastic of Emean,Emin,Emax values had differential diagnosis of benign and malignant prostate diseases;The elastic boundary values of Emean,Emin and Emax for the differential diagnosis of benign and malignant prostate diseases were 41.55 k Pa,34.05 k Pa and 51.20 k Pa,respectively.The diagnostic sensitivities were 66.7%,68.1%and62.5%,respectively;the specificities were 93.7%,90.8%,94.4%,respectively;The best indicator for differential diagnosis of benign and malignant prostate lesions by transrectal ultrasound shear wave elastography was Emean,with a cutoff value of41.55 kPa.Conclusions : 1.The Transrectal ultrasound prostate shear wave elastography has differential diagnosis significance for benign and malignant prostate diseases;2.The best index for differential diagnosis of benign and malignant prostate lesions by transrectal ultrasound shear wave elastography is Emean,and can also reduce the missed diagnosis of abnormality of elasticity but not detected by two-dimensional ultrasound.
