Objective: To evaluate the value of transrectal shear wave elastograpy in diagnosis of prostate cancer and in guiding prostate biopsy by a sysmatic review.Methods: The databases such as PubMed?Embase?Web of Science?The Cochare Library?CNKI?CBM and Wangfang database were searched for chinese and foreign articles,published before December,2016.meanwhile the manual and other retrives were conducted,including relevant literature of transrectal shear wave elastograpy in diagnosis of PCa and in guiding prostate biopsy.The references were screened according to the criteria of diagnostic research.Quality assessment of diagnostic accuracy studies-2 items were used to evaluate the quality of inclued studies.Meta-Disc1.4 statistic software were used to handle data for the selected references.The pooled SSEN,SSPE,PLR,NLR,DOR were calculated.The SROC curve was performed and the area under curves(AUC)was calculated.Stata12.0 statistic software were used to perform the Deeks' funnel plots and investigate the publication bias.Results: A total of 301 studies were searched,Nine articles were included in the present study,involved 1082 suspected prostate cancer patients and 3364 puncture core.1?Six of these literatures were about transrectal shear wave elastography in diagosis of PCa,the SSEN and SSPE in diagnosis of prostate cancer were 0.81[95%CI(0.76,0.84)] and 0.76[95%CI(0.72,0.79)],respectively;the PLR and NLR were 3.94[95%CI(2.80,5.54)] and 0.17[95%CI(0.09,0.33)],respectively;DOR were 26.95[95%CI(11.28,64.39)];AUC was 0.903,SE(AUC)=0.0324.2?Seven of these literatures were about transrectal shear wave elastography in guiding prostatic biopsy,the SSEN and SSPE were 0.87[95%CI(0.84,0.88)] and 0.85[95%CI(0.83,0.86)],respectively;the PLR and NLR were 5.90[95%CI(3.85,9.06)] and 0.14[95%CI(0.06,0.35)],respectively;DOR were 40.83[95%CI(13.19,126.78)];AUC were 0.922,SE(AUC)=0.0276.Conclusion:1?This study shows that transrectal shear wave elastography can effectively improve the detection rate of prostate cancer,its diagnosis performance are obviously higher than that of TRUS,making up for the deficiency of the traditional ultrasound.It plays an important role in the differentiation diagnosis of benign and malignant lesions of prostate,and has a good application prospect in the diagnosis of prostate cancer.2? This study shows that transrectal shear wave elastography has high accuracy in guiding prostate puncture,it can be used to supplemental TRUS prostatic biopsy and be able to effectively reduce the number of unnecessary biopsy.
