Medical Reference Range And Gastric Cancer Early Diagnostic Value Of CEA，CA72-4，CA19-9 In Jincheng

Objective:1. To discuss the distribution of value of serum CEA, CA72-4, CA19-9 among healthy populations in Jincheng city and to establish the medical reference range of them.2. To explore diagnostic value of serum C EA, CA72-4, CA19-9 in single tests and parallel tests of gastric cancer. Methods:1. 971 healthy populations who accord with a standard were selected. They were examined in a upper- first-class hospital in Jincheng from January to December in 2015. The levels of serum CEA, CA72-4, CA19-9 were tested and the medical reference range of them were established using percentiles method.2. 89 patients with gastric cancer who accord with a standard were seleted in the same time and the same hospital as gastric cancer group. 100 healthy populations were randomly selected from 971 as control group. The levels of serum C EA, C A72-4, CA19-9 were tested and ROC curve were drawn, then established the gastric cancer diagnostic value o f serum CEA, CA72-4 and CA19-9 in single tests and parallel tests. Results:1. The average level of serum CEA, CA72-4, CA19-9 among 971 healthy populations were 1.88(1.30, 2.48) ng/ml, 1.78(1.14, 3.23) U/ml, 10.10(6.31, 16.25) U/ml, respectively, described by the median(lower quartile, upper quartile). The medical reference range of serum C EA, CA72-4, CA19-9 were CEA ≤ 3.5ng/ml 、CA72-4 ≤ 6.6U/ml、CA19-9 ≤ 37.0U/ml.2. The levels of serum CEA, CA72-4, CA19-9 in gastric cancer group all had statistically significant difference compared with the control group(P<0.001), and gastric cancer group were higher. The areas under ROC curve of CEA, C A72-4, CA19-9 were 0.684, 0.728, 0.717, respectively. There were no statistically significant difference among them(P>0.05). The cut off point were 2.74ng/ml, 6.10U/ml, 40.83U/ml respectively. Sensitivity of single tests CEA, CA72-4, CA19-9 were 58.4%, 69.7%, 59.6%, respectively. But there were no statistically significant difference among them(P>0.05). Specificities of single tests CEA, CA72-4, CA19-9 were 77.0%, 76.0%, 90.0%, respectively. Specificities of single test CA19-9 was statistically significantly higher than others(P<0.05). And there were no statistically significant difference betwee C EA and CA72-4(P>0.05). Sensitivity of parallel tests was higher than single tests, and C A72-4+CA19-9 parallel test was the best combination. Conclusions:1.The medical reference range of serum CEA, C A72-4, CA19-9 were established, which were similar to the reference kit is given.2.The levels of serum CEA, CA72-4, CA19-9 in gastric cancer patients were higher.3.The best diagnosis of gastric cancer was combination of C A72 4 and CA19-9 parallel experiment. But this method is only used for early screening of gastric cancer. Diagnosis of gastric cancer still need combined with clinical symptoms, imaging examination and other auxiliary diagnosis comprehensive analysis judgment.
