Objective: Continuous monitoring of serum tumor markers in patients withgastric cancer,analyzing Its relationship with the Recurrence、metastasis andSurvival prognosis in patients with gastric cancer,To investigate the clinicalsignificance of tumor markers for gastric cancer patients。Methods: retrospective study was taken in this article.I select272gastriccancer patients with results of serum tumor makers tested preoperative andpostoperative,all the patients were once treated in gastrointestinal surgery in2010. I Collected clinical data and make phone call asking them Postoperativephysical condition.Then i analysis the level of serum tumor makers andcalculate the capability of CEA、CA19-9、CA72-4、CA125for Continuousobservation of the conditions of patient. Analysis of the correlation betweenserum tumor markers and the time of Recurrence,metastasis and survive.Alldata is processed by SPSS19.0.The analysis of the relation of CEA、CA19-9、CA72-4、CA125with TNM staging was completed with correlation test ofnon-parametric. Kaplan-Meier survival analysis was used in analyzing theconnection of CEA、CA19-9、CA72-4、CA125with disease-free survival.Univariate analysis was used to find the factors of3-year survival of gastriccancer. Then these factors were lead into COX multivariate regression model todetermine the independent prognostic factors of gastric cancer.Results:1.The positive rate and the Serum concentrations of CEA、CA19-9、CA72-4、CA125in stage III and IV gastric cancer patients are higher than that in stage I and II,the differences are statistically significant.2.positiverate and the serum level of TM was significantly lower after thegastrectomy,the serum level increased again when recurrence or metastasiswas found, and the serum level was higher than the patients in the same periodwithout Recurrence or metastasis; The sensitivity of Combined detection ofrecurrence91.46%, and specificity was82.35%, The sensitivity of4tumormarkers detected together was significantly higher than four individual test, butthe increase of the false-positive cases made specificity lower.3.If the tumormarks in the serum was positive, then the gastric cancer patient had more likelyto Recurrence and metastasis. disease-free survival was longer in gastric cancerpatient with negative preoperative tumor markers than patients with positivepreoperative tumor markers; age, TNM stage, CEA, CA19-9, CA72-4wereInfluence factors of3-year survival of gastric carcinoma, in univariate survivalanalysis; TNM stage, CA72-4were the independent risk factors of gastriccarcinoma in COX multivariate regression analysis.Conclusion:1.the serum level was higher when the tumor Recurrence ormetastasis,so it had diagnostic value on recurrence or metastasis to detectCEA、CA19-9、CA125and CA72-4in Postoperative, Combined detectioncould increase sensitivity and effectiveness in diagnosing recurrent ormetastasis;2.serum tumor markers can be2.4months earlier than imaging orsome else ways to predict recurrence and metastasis of gastric cancer.3.positivepreoperative CEA、CA125、CA72-4has a relationship with poor prognosis ingastric carcinoma, positive preoperative CA72-4is an independent Risk factorsof prognosis.
