| Objective:This Research will investigate the clinical efficacy of hepatitis B cirrhosis by the syndrome of liver and spleen deficiency, damp heat and blood stasis, treated with nourishing liver, invigorating spleen and eliminating evil. Observing the change of the main clinical symptoms, positive signs, serum test, and abdominal B-ultrasound from all the clinically diagnosed cases is used to verify the effectiveness of the treatment, evaluate the advantages in the treatment standardizedly and then explore the optimal efficiency of combining Chinese with western medicine, improve the life quality of patients, and provide clinical reference for the treatment of hepatitis B cirrhosis.Method:63 patients diagnosed with hepatitis B cirrhosis will be divided into the treatment group (n=38) and control group (n=25) randomly and egually. Antiviral or symptomatic treatment is used in the control group as well as the treatment group which is added with nourishing liver, invigorating spleen and eliminating evil. Differences will be observed before treatment,3 months and 6 months after treatment, such as liver biochemical indexes, liver fibrosis index, blood coagulation index, tumor markers, liver and spleen B-ultrasound, and main clinical symptoms and positive signs. In the end, all collecting data will be counted and analyzed by SPSS 17.0.Results:The effect of symptoms and signs:the treatment group and control group could reduce the patients’performance of exhaustion, lateral thorax pain, anorexia, xerostomia, bitterness, and ascites, and the result was statistically significant (P<0.01). Cmparing with the effect of two groups after treatment, the treatment group could improve the symptoms and signs of patients such like exhaustion, abdominal distention, lateral thorax pain, anorexia, xerostomia, bitterness and ascites(P<0.01), the jaundice and hepatosplenomegaly (P<0.05) with statistical significance.The effect of biochemical function of liver:both groups can reduce the serum ALT, AST, ALP (P<0.01), compared with the control group, the treatment group is superior to reduce ALT (P<0.01), AST, TBIL (P<0.05); Treatment group is more effective than control group, with obvious lower liver cirrhosis ALB and pALB level (P<0.01); after 6 months, the three indexes of liver fibrosis in the two groups are decreased (P<0.05), in reducing the PC III and IV-C, treatment group is superior to control group (P<0.05). In the improvement of coagulation indexes, the treatment group can shorten the PT and improve the PTA and PLT (P<0.01), meanwhile, the control group only to shorten the PT(P<0.05), so the treatment group is superior in improveing the PT and PLT (P<0.05) than control group; among tumor index, both groups could decrease AFP level (P<0.05) and the treatment group is better than the control group (P< 0.01). The influence of liver and spleen B-ultrasonography:two groups improve the right hepatic pitched blade diameter, diameter of portal vein with no statistical significance (P>0.05), the treatment group can shorten the inside diameter of splenic vein (P<0.05), the effect is better than the control group (P<0.05). Evaluation of the total efficiency:the total efficiency of the treatment group is 89.47%, of the control group is 72%, two groups have significant difference (P<0.05), therefore the efficiency of the treatment group is higher than that one of the control group.Conclusion:The both groups could treate hepatitis B cirrhosis effectively, and the treatment group can better improve all adpects than the control one, such as the symptoms and signs, liver biochemical indexes liver fibrosis index, blood coagulation index, a-fetoprotein, liver and spleen B-ultrasound, and main clinical symptoms and positive signs. In a word, the therapy of nourishing liver, invigorating spleen and eliminating evil combined with western medicine treatment can better delay the liver fibrosis, protect the liver cells, relieve the clinical symptomsd and own a positive curative effect. |
