Abstract/Summary:
Objective:Observe the clinical effects of electro acupuncture stimulation on myasthenia of mechanical ventilation patients with sepsis/multiple organ dysfunction syndrome.Methods:46 mechanical ventilation patients with sepsis/multiple organ dysfunction syndrome were randomly divided into the experimental group and the control group,41 patients complete the whole research process, fall off, excluding cases in 5 cases. Treatment group 24 cases, fall off, eliminate in 2 cases, the actual observation of 22 cases, control group 22 cases, falls off,3 cases,19 cases actual observation. The control group received Sepsis basic support treatment, and the experimental group received the treatment of electroacupuncture on the treatment basis of the control group (Jianyu point and Quchi point in Bilateral upper limb,Biguan point and Zusanli point in Bilateral lower limb),twice a day, each time for 30 minutes. The beginning of the treatment was the first time to receive electro acupuncture stimulation, and the end was the time of Ventilator weaning success. Results:Before treatment, there was no statistically significant difference in the upper arm circumference, thigh circumference, the thickness of the biceps brachii, oxygenation index, ventilator trigger sensitivity between the two groups (P>0.05).At the end of the treatment, the upper arm and lower limbs in varying degrees after treatment in the two groups, but in control group was more obvious test group arm perimeter on day 3 (25.89± 1.98 vs 26.45±1.78) after lower limb circumference in the 5 days (44.02±1.97 vs 44.51± 2.13) when compared with the control group there was a significant difference (P< 0.05).After treatment of the two groups of biceps brachii muscle thickness in 3 days ago without significant difference (13.57±0.81 vs 13.30±0.66; 13.12±0.73 vs 13.21±0.76, p >0.05), but in the 5 days compared with statistically significant differences (12.71± 0.88vs 13.09±0.82; 12.46±0.97 vs 13.00±0.90; p<0.05).There was no significant difference in Lovett score of both groups before treatment (P>0.05).At the end of the treatment, muscle strength was significantly improved than before in the experimental group, and there was significant difference in Lovett score of both groups (2.67±1.222.55± 1.50; 2.67±1.22 vs 2.67±1.22; 2.75±1.01 vs 2.67±1.22),Compared with the control group, the same period has obvious statistical difference (2.55±1.50 vs 2.75±1.01; 2.59± 1.34 vs 2.89±1.41;2.67±1.22 vs 3.05±1.50; 2.75±1.01 vs 3.35±0.93; P<0.05).After electroacupuncture treatment, the two groups of patients with oxygen synthetic index trends and changes in thickness of the biceps brachii was positively related to in 5 days compared with significant difference (177.54±13.24 vs 189.87±10.77; 190.33±10.18 vs 215.92± 11.81; p< 0.05).Two groups of patients with mechanical ventilator triggering sensitivity and the thickness of the biceps brachii, oxygen synthetic index showed a negative correlation, in the 5 days showed a decreasing trend, is more obvious in the experimental group, compared with the control group, significant difference compared with the (4.01±0.95 vs 4.61± 1.12; 3.45±1.02 vs 4.11±0.97, P< 0.05). Test groups of patients with mechanical ventilation, ventilation time (8.35±1.13 vs 10.34±2.12), and ICU hospitalization (11.01±1.78 vs 16.33 ±1.31) times were significantly shorter than the control group (P< 0.05). There was no significant difference in mortality rate between the two groups (P=0.443> 0.05) compared with the 28 groups by using the Kplan-Meier method. Conclusion:Electro acupuncture stimulation on mechanical ventilation patients with sepsis/multiple organ dysfunction syndrome can prevent muscle atrophy and muscle weakness, and can improve the level of oxygenation of patients, shorten the time of mechanical ventilation and ICU length of stay, but can not reduce the 28 day mortality.
