Mechanical ventilation (MV) is a lifesaving therapy with a myriad of organ-system complications that can significantly affect the outcome of critically ill patients. The most prevalent GI complications associated with MV are stress ulcer and GI hypomotility. GI complications can occur in up to 50% of patients receiving MV. Acute respiratory failure requiring MV for >48 h has been shown to be one of the two strongest independent risk factors for clinically important GI bleeding in the ICU. Meakins and Marshall showed that the gut is a potential trigger of multiple organ dysfunction syndrome. In the last decade, alterations at the cellular level have become a focus of study as several investigators have suggested a role for altered gut barrier function in the pathogenesis of MODS. Chinese medicine is an important role for protection GI function and prevention GI complications in patients receiving mechanical ventilation.The thesis mainly includes literature review and study. The literature reviews consist of two parts. The first one summarizes several mechanisms to explain how MV unfavorably affects the GI tract, splanchnic hypoperfusion appears to be particularly important. The second one summarizes the development of prevention and treatment of GI complications and MODS by traditional Chinese medicine.In the clinical study, we investigate the effect of dachengqi decoction on the GI in patients receiving mechanical ventilation. The patients receiving mechanical ventilation in our ICU were selected in this study. All patients were randomly divided into dachengqi decoction group and non- dachengqi decoction group. The patients were treated with dachengqi decoction 100ml a day in treatment group. Except dachengqi decoction, the other therapies were similar between the two groups. Results: The incidence of stress ulceration, gastrointestinal paralysis and multiple organ dysfunction syndrome were obviously lower in treat group than that in non-treat group. The mortality rate in 28 days were obviously lower in treat group. Conclusions: The dachengqi decoction have protective effect on GI complications in patients receiving mechanical ventilation and also can prevent the incidence of stress ulceration, gastrointestinal paralysis and multiple organ dysfunction syndrome , decrease the mortality rate in 28 days in critical illness.
