Observation Of Qiangxin Preparation Clinical Effect On CHF

Objective:This study is based on the standardized treatment of chronic heart failure with Qiangxin preparation to observe the changes of New York heart association, Minnesota Living With Heart Failure Questionnaire (MLHFQ), signs and symptoms of TCM scale,6-minute walk test (6MWT), B-type natriuretic peptide (BNP), ultrasonic cardiogram, readmission rate and Adverse Event. To provide the theory foundation for using traditional Chinese medicine to treat the patients of chronic heart failure by exploring the efficacy and safety of the Qiangxin preparation.Method:60 cases of chronic heart failure patients who met the inclusion criteria from Jiangsu Province Traditional Chinese Medicine Hospital. All patients were randomly divided into 2 groups, each of 30 patients, and there was no significant difference in baseline data, The control group was treated with standard treatment through the Chinese guidelines on diagnosis and treatment of heart failure 2014, treatment group was also, and combined Qiangxin preparation. (Composition:Zhi Huangqi 20g、Dang Fupian 10g、Dang Shen 20g、Dan Shen 20g、Che Qiancao 20g、Ting Lizi 20g, Mai Dong 10g、Yu Zhu 10g). And according to the specific circumstances of the patient to add and subtract the traditional Chinese medicine and dosage. All patients drank a dose of Chinese herbal medicine about 200 ml per day, and the course of treatment was 6 months. Related indexes were observed at 3 and 6 months after treatment, Including cardiac function, symptoms and signs of TCM, MLHFQ,6MWT, BNP, ultrasonic cardiogram, readmission rate and Adverse Event. To compared these data with statistical methods.Results:1. Before treatment, the data of all patients have no significant difference, including basic information, heart failure symptoms and Auxiliary examination.2. After treatment, the two groups have significant improvement in New York heart association, Chinese medicine symptom complex score and MLHFQ, the difference was statistically significant (P<0.05). After 6 months of treatment, the improvement was better than 3 months after the treatment. Simultaneously, the improvement of the treatment group was more obvious than that of the two groups (P<0.05).3.6MWT and BNP in both groups improved with significant difference in treatment group (P< 0.05) and without in control group (P> 0.05). Comparison between the two groups, the treatment group was more effective (P< 0.05).4. After 3 months of treatment, the two groups had a bit of improvement in LVEF, but no statistical difference (P>0.05). LVEF further increase when treatment reached 6 months, and the curative effect of the treatment group was statistically significant compared with that before treatment (P>0.05). Before and after treatment, LVIDd, LVIDs, EDV and ESV were not changed significantly. There was no statistical difference between the two groups in the 3 months and 6 months after treatment (P>0.05).5. There were no obvious adverse drug reactions in the two groups of patients. And monitoring of vital signs and liver and kidney function before and after treatment showed no significant abnormal changes.Conclusion:1. Compared with the standardized treatment of pure western medicine, the combination of Qiangxin preparation can further increase the therapeutic effect. Including the improvement of heart failure symptoms, heart function, exercise tolerance and life quality.2. All patients were not found may be related to the abnormal changes in drugs by monitoring the safety index In the course of 6 months of treatment. It shows that Qiangxin preparation is safe and effective for patients with chronic heart failure. So we can be widely used in clinical practice.
