Object:to treat IBS-D patient with acupuncture of regulating mentality and spleen strengthening; to evaluate effectiveness of acupuncture of regulating mentality and spleen strengthening in IBS-D treatment with the western medicine, pinaverium bromide (dicetel, manufactured by Abbot) as the reference and the internationally recognized IBS-SSS as standard for objective assessment; and to observe influence of acupuncture of regulating mentality and spleen strengthening toward MTL in serum by comparing serum MTL change of IBS-D patients after acupuncture treatment with serum MTL after medication.Method:samples are randomly divided into groups by centralized stochastic system.43 patients are randomly divided into regulating mentality and spleen strengthening acupuncture group (the experimental group) that involves 23 patients and western medicine pinaverium bromide group (the control group) that involves 20 patients. Patients in the acupuncture group are treated with acupuncture of regulating mentality and spleen strengthening for 3 times per week. Patients in the western medicine group are medicated with pinaverium bromide (dicetel, manufactured by Abbot,50mg, tid). All patients are treated for 6 weeks. Observation include the following contents:(1) to observe and evaluate curative effect of regulating mentality and spleen strengthening acupuncture and pinaverium bromide to IBS-D by comparing IBS-SSS symptoms before and after treatment with IBS-SSS questionnaire that the two groups of patients fill after the base line phase and the sixth week of treatment; (2) to observe influence of acupuncture and western medicine toward serum MTL of patients and correlation between improved IBS-D symptoms and serum MTL by testing serum MTL of the two groups of patients under empty stomach status before and after the treatment.Result:1. Compared with total score of IBS-SSS symptom in the base line phase, patients in the acupuncture group are generally found with significant improvement in IBS-SSS symptoms score in the sixth week (P<0.05); the acupuncture treatment is also found with significant improvement in symptom scores including stomachache, duration of stomachache, abdominal distension extent, defecation comfort and influence of IBS-D toward daily life in IBS-SSS (P<0.05).2. Compared with total score of IBS-SSS symptom in the base line phase, patients in the western medicine group are generally found with significant improvement in IBS-SSS symptoms score in the sixth work (P<0.05); the western medicine treatment is also found with significant improvement in symptom scores including stomachache, duration of stomachache, abdominal distension extent, defecation comfort and influence of IBS-D toward daily life in IBS-SSS (P<0.05).3. Total IBS-SSS score after the entire treatment indicates that curative effect of acupuncture is no significant difference(P>0.05), and patients in the acupuncture group are found with better satisfaction about defecation comfort and influence of IBS-D toward daily life (P<0.05) while patients in both two groups are found with no significant difference in scores of stomachache, duration of stomachache and abdominal distension extent (P>0.05).4. Compared with serum MTL value of the two groups of patients measured before the treatment, both acupuncture treatment and pinaverium bromide treatment are found with improvement in serum MTL concentration of the patients (P<0.05); no significant difference is found between extent of serum MTL decrease caused by the two methods.Conclusion:1. IBS-SSS symptom is significantly improved after acupuncture treatment; patients in the acupuncture group are found with significant improvement in stomachache, duration of stomachache, abdominal distension extent, defecation comfort and influence of IBS-D toward daily life in IBS-SSS..2. Regulating mentality and spleen strengthening acupuncture may significantly reduce serum MTL concentration of IBS-D patients; acupuncture treatment may improve patients’ IBS-D symptom by reducing serum MTL concentration of patients.
