| Objective0 grade diabetic foot (DF) [1] refers to the potential risk of foot ulcers, but still of no ulcers. With main symptom of insufficient blood supply to the lower extremities, cold skin and (or) rest pain, intermittent claudication, purple dark, numbness, or loss of sensation, and even severe cases, DF can affect the quality of life of patients, increase the economic burden. Therefore, early intervention of its development, active treatment 0 grade DF has become a focus of clinical research. With a series of distinctive advantages of little side effects, easy operation, easy access to material, significant improved symptom and slowing down disease progression, the traditional Chinese way is increasingly used in the treatment of 0 grade DF aspects. Through this research, a simple, available method can be provided by observing QIDAN external fumigation-washing prescription of 0 grade DF treatment efficacy and safety, and exploring its mechanism of action, clinical treatment 0 grade DF.Methods90 cases (criteria 0 grade DF) were randomly divided into experimental group 1 (QIDAN external Fumigation-washing therapy), group 2 (QIDAN Washing side Fumigation+epalrestat oral tablets) and control group (Warm water soaking+according to par department his oral). N= 30, to the standard treatment for 4 weeks, during which basic treatments (including blood pressure, blood glucose, etc.) were maintained during the same original program, due to the fact that stopping taking it may affect the development and prognosis of 0 grade DF drugs. Before and after treatment, EMG line, ankle-brachial index (ABI) and blood rheology and other tests should be taken, also, symptom score rating scale score, and TCM should be used. The results of comparison treatment changes in inspection and improvement of symptoms before and after can verify this clinical research program efficacy. Researchers should note that the foot skin temperature color, etc., liver and kidney index, ECG, etc. in order to observe its security.Results1. Symptom score changes and effect:observing the results, there is no significant difference among the aforementioned three groups (P> 0.05), but there was a significant difference of each symptom scores before and after treatment (P<0.05); and in improving the efficacy of clinical symptoms context, the experimental group total effective rate of group 1 was 86.67%, group 2 was 93.33% in the control group was 86.67%, and pairwise comparisons showed no significant difference (P> 0.05).2. The effect and change of the lower limbs and ABI:Each group themselves ABI values have significantly increased after treatment (P<0.05);by observing the effect of the lower limbs ABI change, we can see the left leg three groups total effective rate were 80.00%,90.00%,76.67%, respectively. Total effective rate of right leg three groups were 83.33%,93.33%,80%. Pairwise comparison among the three groups, the experimental group 2 and group 1, control group were significant difference (P<0.05), but the experimental group 1 and the control group did not differ significantly (P> 0.05).3. The results and efficacy of lower limbs EMG:each lower extremity tibial nerve, common peroneal nerve conduction velocity were significantly increased after treatment (P<0.05);the results of nerve conduction velocity of three groups of left tibia, left and right Philippians:the total effective rate was 80%,80%,83.33%, respectively (experiment D.And 90%,90%, 83.33%, (experiment group 2)76.67%,80%,80%, (the control group). By pairwise comparison, the experimental group and the other 2 groups were significant difference (P<0.05), there was not significantly different (P> 0.05) between the experimental group 1 and the control group; the efficacy of the right tibial nerve conduction velocity was improved:the above three groups of the total efficiency of 80%,93.33%,76.67%, respectively, and the rest of the experimental group had no significant difference between two groups (P> 0.05), there was significant difference between the experimental group 2 and the control group(P<0.05).4. Comparison of blood rheology before and after treatment:among the top three groups there was no distinctive difference among high shear, low shear whole blood viscosity and plasma viscosity (P> 0.05); aforementioned three elements in experiment 1 showed little change (P> 0.05), high shear whole blood viscosity, low shear viscosity was significant different (P<0.05) which was hugely different in the other two groups before and after treatment, but no significant difference in plasma viscosity (P> 0.05).ConclusionQIDAN external fumigation-washing prescription of 0 DF treatment could significantly improve the clinical symptom score, ABI, and nerve conduction velocity of patients, but insignificantly decrease hemodynamic indexes. The effects to improve the clinical symptoms of the three groups are equally in improving the ABI, nerve conduction velocity in efficacy, and the combination of western medicine group and Chinese traditional group was significantly effective than each separate group, also, TCM group and western medicine group affect almost the same. But the TCM group gastrointestinal side effect is little, the operation is convenient and easy to be accepted, is worth the clinical promotion. |
