[Purpose]Through research on the Dali Prefecture of access to mental health service, quality of life of severe mental patients, access to mental health service influence on quality of life of severe mental patients in order to understand and analyze the relevant local mental health service needs, financial access, geographic access, mental health resources, influencing factors on quality of life of severe mental patients, provide the reference for the local government to improve the access to mental health service and quality of life of severe mental patients.[Method]Investigation for the second people’s hospital of Dali Prefecture, taking the residence in the Dali Prefecture and in accordance with the diagnostic criteria of《International Classification of Diseases》(ICD-10) belongs to the category of severe mental patients as the research object of the field survey and data collection. The research on access to mental health service using self-made “Questionnaire for Access to mental health service”. The research on quality of life of severe mental patients using “Quality of life Questionnaire for Psychiatric patients ”. For access to mental health service mainly through the issues related to the option of constituent ratio analysis; quality of life of severe mental patients mainly uses t test, analysis of variance, multiple linear regression analysis method to carry on the analysis; access to mental health service influence on quality of life of severe mental patients by multiple linear regression analysis method and Spearman’s rank correlation coefficient analysis method. Related data and summary by Epi Date3.1, data analysis using Spss17.0.[Results]1.General Information1.1Recall questionnaire: In this survey 427 questionnaires were distributed, 348 questionnaires were recovered, the recovery rate was 81.50%, the effective questionnaires were 317, and the effective rate was 91.09%.1.2The reliability and validity of the questionnaire: “Questionnaire for Access to mental health service” reliability analysis, at all levels and questionnaires overall Cronbach coefficient between 0.494 ~ 0.827; “Questionnaire for Access to mental health service” validity analysis excluded load of less than 0.45 option, the questionnaire meet the needs of the survey.1.3Demographic characteristics of object: Male and female sex ratio was 1: 0.8; Han and Bai are the majority(91.17%); age between 20 to 49 years old young are the majority(74.49%); unmarried and married ratio was 1:1.1; primary school and below and junior high school education are the majority(76.34%); no occupational patients are the majority(92.11%); rural patients are the majority(87.70%).2. Access to mental health service2.1Mental health service needs: 61.83% of severe mental patients need the appropriate mental health services; 82.65% of severe mental patients considered the mental health knowledge is important;33.44% of severe mental patients can have timely access to mental health knowledge; 55.84% of severe mental patients with first onset to treatment in a timely manner, 72.87% of severe mental patients initial medical institutions to county, district level above psychiatric hospital, 60.88% of severe mental patients think that the initial medical institution treatment is good,65.30% of severe mental patients were able to obtain the desired therapeutic effect in a psychiatric hospital;38.49% of severe mental patients can continuity accessible patient treatment in a psychiatric hospital.2.2Financial access: 70.66% of severe mental patients with average monthly household income concentrated in the 1500 yuan or less; 99.37% of severe mental patients to purchase the appropriate medical insurance; 24.61% of severe mental patients think psychiatric hospital medical costs are not cheap, 21.45% of severe mental patients think the treating diseases costs are small degree of influence to the family economy.2.3Geographic access: The distance of severe mental patients family nearest medical institutions that the village, community clinics and township hospitals are the majority;66.56% of severe mental patients arrive the nearest medical institutions focused on the distance of 3 kilometers or less; 72.23% of severe mental patients arrive the nearest medical institutions focused on the time of 30 minutes or less; The severe mental patients to psychiatric hospital distance(km) M is 66, Q is 56; The severe mental patients to psychiatric hospital time(hours) M is1.5, Q is 1.7.3. Quality of life of severe mental patients Quality of life of severe mental patients score is 103.12 ±21.82, Multiple linear regression analysis showed that the gender, marriage, education, occupation and place of residence are factors affecting the quality of life of Severe mental patients.4. Access to mental health service influence on quality of life of severe mental patients Multiple linear regression analysis showed that “First onset and treatment timely”,“initial medical institutions”, “initial hospital treatment effect”, “psychiatric hospital treatment effect of acquisition”, “family average monthly income”, “psychiatric hospital medical expenses”, “the nearest medical institutions in time”, “mental health resources” have positively correlated with the quality of life of severe mental patients,“continuity of treatment in psychiatric hospital” has negative correlation with the quality of life of severe mental patients.[Conclusion]1.Demand side of access to mental health service:Dali Prefecture severe mental patients for mental health services needs is high; High degree of understanding of the importance of mental health knowledge, but The timely acquisition of mental health knowledge needs to be improved; Severe mental patients with early stage of disease treatment delay is more serious, but the way of treatment and behavior is better;Severe mental patients with psychiatric hospital treatment effect is good, but the status of the continuity of treatment in psychiatric hospital needs to be improved.Dali Prefecture severe mental patients to buy insurance rates high, psychiatric hospital medical expenses is reasonable, but the family income is relatively low, disease treatment costs to the family economy influence is relatively large.2.Supply side of access to mental health service:The distance of Dali prefecture severe mental patients family nearest medical institutions that the village, community clinics and township hospitals are the majority, the geographical access of nearest medical institutions is good.The Dali prefecture mental health institutions have a lot of beds and medical staff, but the resources are too concentrated.3.The quality of life of severe mental patients in Dali prefecture is better. The important factors of access to mental health service influence on quality of life of severe mental patients in Dali Prefecture are “First onset and treatment timely”,“initial medical institutions”, “initial hospital treatment effect”, “psychiatric hospital treatment effect of acquisition”, “family average monthly income”, “psychiatric hospital medical expenses”, “the nearest medical institutions in time”, “mental health resources”.
