Objective:To explore the clinical significance of treating lumbar disc herniation by spinal manipulation.Methods:178 LDH patients in hospital were randomly divided into observation group (89 cases) and control group (87 cases). In addition to receive the same dehydration, anti-inflammatory and other conventional baseline treatment, observation group were treated with Feng’s spinal manipulation, while control group received fasle spinal manipulation. During observation period of 20 days clinical data were collected as follows: symptom assessment (ODI), nerve root irritation signs (RS) score, the structural disorder symptoms (PS) score, and MRI measurement of conditions for the protruded nucleus pulposus in spinal canal.Results:After 20 days’treatment, ODI, RS score and PS score improved to a certain extent in two groups. In the observation group, both ODI index and RS score were significantly better than that of control group (P<0.01, P<0.01), while PS score between the two groups was no statistically significant difference (P>0.05). On the other hand, MRI had no significant change before and after treatment.Conclusion:Correcting "vertebral dislocation" with spinal manipulation in the treatment of lumbar disc herniation has certain promoting effect on relieving symptoms. But the symptoms change and "nucleus" morphology and structure changes have no significant correlation. The results show that the key of clinical mechanism with spinal manipulation is the recovery of function and the establishment of compensatory balance.
