Part1 The application research of enhanced 3D-FIESTA-c sequence MRI in displaying the preganglionic nerve of the brachial plexusBackground Preganglionic nerve injury of brachial plexus is a clinically common type of injury, serious threat to human health, without the possibility of recovery, if not diagnosed promptly, the patient will be lifelong disability. Especially in recent years, the incidence of the disease is on the rise. It’s desirable to correctly and effectively assessing brachial plexus injury in the early stage.3D-FIESTA-c series as a kind of MRM imaging technology has a lot of advantages compared to other methods. The purpose of this study was to evaluate the feasibility and values of enhanced MRI using 3D-FIESTA-c sequence in displaying the preganglionic nerve of the brachial plexus, which help its early clinical diagnosis and treatment.Objective To disscuss the feasibility and values of enhanced MRI using 3D-FIESTA-c sequence in displaying the preganglionic nerve of the brachial plexus.Methods 20 volunteers and 10 patients suffering from brachial plexus preganglionic injury underwent MRI using 3D-FIESTA,unenhanced and enhanced 3D-FIESTA-c sequences, respectively.The displaying number of the nerve bundle of anterior and posterior rootlets of brachial plexus contrast noise ratio(CNR)and artifacts of images at the three sequences were evaluated.The signs of the normal brachial plexus preganglionic part and its lesions at contrast enhanced MRI using 3D-FIESTA-c sequence were observed.Results 3D-FIESTA,unenhanced and enhanced 3D-FIESTA-c in number of the nerve bundle of anterior and posterior rootlets of the preganglionic nerve of the brachial plexus showed statistically significant. Unenhanced and enhanced 3D-FIESTA-c in number of the nerve bundle of anterior and posterior rootlets of the preganglionic nerve of the brachial plexus were more than 3D-FIESTA-c.And unenhanced and enhanced 3D-FIESTA-c in number of the nerve bundle of anterior and posterior rootlets of the preganglionic nerve of the brachial plexus have no statistical significance. The CNR of three sequences above were 39.44±6.65、39.91±6.52、25.92±6.95,respectively.The CNR of 3D-FIESTA, unenhanced and enhanced 3D-FIESTA-c in the preganglionic nerve of the brachial plexus showed statistically significant. The CNR of 3D-FIESTA,unenhanced 3D-FIESTA-c were more than enhanced 3D-FIESTA-c. And the CNR of 3D-FIESTA and unenhanced 3D-FIESTA-c have no statistical significance. Enhanced sequence 3D-FIESTA-c increase the muscle in the tissues of the background signal strength, unfavorable to observe the normal brachial plexus preganglionic part and its lesions.Conclusion Although unenhanced and enhanced 3D-FIESTA-c in number of the nerve bundle of anterior and posterior rootlets of the preganglionic nerve of the brachial plexus have no statistical significance.But due to their increased muscle in the tissues of the background signal strength, the author thinks that should be good at 3D-FIESTA-c scanning before the enhancement.Part2 The preliminary research of unenhanced 3D-FIESTA-c sequence in displaying the compression of lumbosacral nerve root caused by lumbar disc herniationBackground Lumbar disc disease is a clinical common disease, often by intervertebral disc degenerative change, excessive load and chronic strain, etc.Patients often accompa-nied by lumbar disc prolapse of lumbosacral nerve root compression.In recent years, magn-etic resonance imaging (MRI) 3D-FIESTA-c sequence development mature, has been widely used in spinal nerve imaging.Objective To evaluate unenhanced 3D-FIESTA-c in the detection of compression of nerve roots caused by lumbar disc herniation.Methods 3D-FIESTA-c images in 20 cases with lumbar disc herniation were analyzed.Results 3D-FIESTA-c image showed low signal on lumbosacral nerve root.3D-FIESTA-c image demonstrated compression of only one nerve root in 66.7% of disc herniations, compression of two spinal nerve roots in 16.7%, compression of multiple nerve roots in 6.7%, and compression of only dural sac in 9.9%.Conclusion The unenhanced 3D-FIESTA-c sequence could demonstrate the course of lumbosacral nerve root and compression of nerve roots caused by disc herniation.
