| Objective:Observation of taking gu shang fu yuan soup in leg open fracture of free bone block replantation postoperative clinical curative effect.Methods:30 patients who had leg open fracture and corresponded with the diagnostic criteria and inclusion criteria, randomized single blind method, according to the order of admission were randomly divided into treatment group 15 cases,15 cases in the control group. All patients were performed on wound debridement and internal fixation or external fixation plus free bone particles which were processed by platelet rich plasma (PRP) back to graft and implantation antibiotic bone cement chain. The patients in the treatment group in the first day after the operation began to take the gu shang fu yuan soup, taking 6 consecutive weeks and the control group was not. All patients in the postoperative second week, the third week were again injected with PRP in the original site of bone particles replantation. Postoperative pain score of two groups in the second day after operation, the fourth day and the seventh day were assessed and recorded. All patients with limb swelling scores in the first day after surgery, the third day, the seventh day were measured and recorded. From fasting venous blood in the first day and the seventh day after operation were collected in the morning, and then measured and recorded their blood rheology indexes. The content of alkaline phosphatase (ALP) in the serum of the two groups in the first day after operation, the second week, the third week and the fourth week were measured and recorded. The patients in the postoperative sixth week, the tenth week, the sixteenth week and the twenty-fourth week were required to take X-ray, so that the experimenter could assess and record the bone callus growth of their limb. Recorded two groups of patients with fracture healing time. Results:After 10 months of follow-up, all patients completed the follow-up. All cases of fracture healed and the healing time was 4-8 months, an average of 6.1 months.Wound infection happened in 3 cases,which healed after sensitive antibiotic therapy. There were no statistieally significant differences in pain VAS score among the two groups on the second postoperative day and the fourth day,and on the seventh day,the experimental group’s pain VAS score was significantly lower than that of the control group with statistical significance(P<0.05). The limb swelling scores in the experimental group and the control group had no statistical significance on the first postoperative day and the third days.Seventh days after surgery, experimental group’s limb swelling scores were significantly lower than that in the control group(P<0.05). There were no statistieally significant differences in hemorrheologic outcome among the two groups on the first postoperative day.And there were statistically significant differences on the 7th day after therapy(P<0.05). The ALP levels in the experimental group and the control group on the first postoperative day had no difference. And the level of serum ALP in experimental group were higher than those in control group at the postoperative second week, third week and fourth week (P< 0.05). Higher than the control group in the sixth week, the tenth week, the sixteenth week and the twenty-fourth week of experimental group score reflect bone callus growth; The fracture healing times of experimental group were lower than those of the control group, and the comparison between them showed significant differences (P<0.05).Conclusion:Gu shang fu yuan soup could reduce postoperative pain, reduce patients suffering from limb swelling degree, raised leg open fracture free bone block replantation postoperative serum alkaline phosphatase levels, improvement of microcirculation in bone replantation site, promote to graft bone callus growth and shorten fracture healing time.
