ObjectiveTo investigate the effect of Yam gruel on bifidobacterium(bifidobacterium longum, bifidobacterium breve,bifidobacterium adolescentis,bifidobacterium infantis,bifidobacterium total bacteria)in the gut and blood sugar(fasting blood glucose,2h postprandial blood gluc-ose)in patients with type 2 diabetes.so as to provide new targets for the prevention of diabetes from the angle of the intestinal flora.Meanwhile.the results may provide references and evidences for the curative effect of yam gruel in the diabetic population.MethodsAccording to incorporated into and exclusion criteria,90 patients of type 2 diabetes were recruited in this clincal trail.The research object were randomly divided into trial group and control group (n=45)according to the random number table method.The control group were received routine health education and dietary guidance.While those patients in the trial group received yam gruel for 12 weeks except of routine health education and dietary guidance. All research object prohibit antibiotics, prebiotics and probiotics products during the intervention. Respectively before the intervention, intervention of 8 and 12 weeks.observed two groups of bifidobacterium(bifidobacterium longum, bifidobacterium breve, bifidobacterium adolescen-tis,bifidobacterium infantis.bifidobacterium total bacteria)in the gut and blood sugar(fasting blood glucose,2h postprandial blood glucose).And make statistical analysis.Results1.Baseline ComparisonThere were no significant difference in general condition(age,sex and educational level,etc)and basic clinical data (course of disease,medication and complication.etc) between the two groups before the trial (P>0.05).And there were no significant difference in Bifido-bacterium longum,bifidobacterium breve.bifidobacterium adolescentis, bifidobacterium infa-ntis.bifidobacterium total bacteriain the gut, fasting blood glucose and 2h postprandial blood glucose(P>0.05).2.Effect of Intestinal floraComparison within group:(1)After 8 weeks of the trial,the result of PP analysis and ITT analysis in the trial group were that the quantity of bifidobacterium longum and bifidobacteri- um adolescentis were increase than that before the trial(P<0.05).While,bifidobacterium longum, bifidobacterium breve,bifidobacterium adolescentis, bifidobacterium infantis and bifidobacterium total bacteria in the control group were no statistically significant differen-ce (P>0.05).(2)After the trial and before the trial,according to PP analysis and ITT analysis the quantity of bifidobacterium adolescentis, bifidobacterium infantis and bifidobacterium total bacteria had statistical difference in the trial group(P<0.05).Especially, bifidobacterium adolescentis had significant difference(P<0.01).There were no significant difference in bifid-obacterium longum, bifidobacterium breve,bifidobacterium adolescentis.bifidobacterium inf-antis and bifidobacterium total bacteria in control group by PP analysis (P>0.05).But ITT analysis,bifidobacterium adolescentis also had significant difference in control group (P< 0.05).And there were no significant difference in bifidobacterium longum, bifidobacterium breve,bifidobacterium infantis and bifidobacterium total bacteria in control group(P>0.05). (3)After the trial and after 8 weeks of the trial, only bifidobacterium adolescentis had significant difference in the trial group by PP analysis and ITT analysis(P<0.05). Meanwhile, there were no significant difference in bifidobacterium longum, bifidobacterium breve, bifido-bacterium adolescentis,bifidobacterium infantis and bifidobacterium total bacteria in control group(P>0.05).Comparison between groups:(1)After 8 weeks of the trial.there were statistical difference in bifidobacterium longum and bifidobacterium adolescentis between two groups by PP anal-ysis and ITT analysis(P<0.05).(2)After the trial, the trial group compared with control group, bifidobacterium adolescentis and bifidobacterium infantis had a difference(P<0.05)by PP an-alysis.And ITT analysis, there were significant difference in bifidobacterium adolescentis, bifidobacterium infantis,bifidobacterium total bacteria(P<0.05).3.Effect of blood glucoseComparison within group:(1)After 8 weeks of the trial and before the trial,the result of PP analysis and ITT analysis in the trial group were that fasting blood glucose and 2h postprandial blood glucose both had significantly decreased.the difference was statistically significant (P<0.01).(2)After the trial and before the trial,significant changes in fasting blood glucose and 2h postprandial blood glucose in the trial group(P<0.01).(3)After the trial and after 8 weeks of the trial, according to PP analysis and ITT analysis,there were significant difference in fasting blood glucose and 2h postprandial blood glucose in the trial group (P<0.01).Comparison between groups:(1)After 8 weeks of the trial.there were no statistical difference in fasting blood glucose and 2h postprandial blood glucose between trial group and control group by PP analysis(P>0.05).According to ITT analysis,fasting blood glucose had statistical difference(P<0.05)between trial group and control group.Nevertheless,compared with the control group,2h postprandial blood glucose in trial group had no statistical diffe-rence(P>0.05).(2)After the trial,the result of PP analysis showed that fasting blood glucose and 2h postprandial had statistical difference(P<0.05).The difference of fasting blood glucose and 2h postprandial blood glucose between trial group and control group were significant by ITT analysis(P<0.01).4.Correlation between bifidobacterium in the gut and blood glucoseThere were negative correlation between bifidobacterium adolescentis and fasting blood glucose(r=-0.104,P<0.05).Meanwhile,bifidobacterium adolescentis had a negative relation-ship with 2h postprandial blood glucose(r=-0.149,P<0.05).Bifidobacterium total bacteria also had a negative relationship with fasting blood glucose(r=-0.109,P<0.05).Conclusion(1)Yam gruel can promote the content of intestinal bifidobacteria in patients with type 2 diabetes,so as to provide references and evidences for the prevention of diabetes from the angle of the intestinal flora.(2)Yam gruel can reduce fasting glucose and 2h postprandial blood glucose after breakfast.so it could be as an aid in the treatment of patients with type 2 diabetes.(3)There were negative correlation between bifidobacterium adolescentis and fasting blood glucose.Meanwhile.bifidobacterium adolescentis had a negative relationship with 2h postprandial blood glucose.(4)Bifidobacterium total bacteria also had a negative relationship with fasting blood glucose.
