Background:Rotator cuff tear is a main cause of pain and dysfunction of shoulder joint, which has a strong impact on the normal lives of the patients. Although both the surgical and non-surgical treatment can be used as a routine for the patient with full-thickness rotator cuff tear, there’s still no appropriate prediction mechanism to judge the tendency of the patient accurately and quickly. Therefore, seeking for a reliable evidence to predict the tendency of the treatment approaches of patients and analyzing factors concerning that for patient with rotator cuff tear is beneficial to increase the efficiency of medical workers, reduce medical costs, provide more customized and reasonable advice for patients, decrease medical conflict and improve medical quality.Objective: Seek for a reliable evidence to predict the tendency of the treatment approaches of patients who face the choice in the surgical or non-surgical treatments, and analyze factors concerning that for patients with rotator cuff tears.Methods: Randomly picked 392 patients with rotator cuff tear(full-thickness tear)in our hospital, and their information were collected retrospectively, including age, sexuality, body mass index(BMI), smoking history, the extent of the rotator cuff tear, the United States shoulder elbow surgery score(ASES)Using regression analysis to study the connection between the characteristics with the appeal and treatment approaches for patients.Results: Among the 392 patients,224 were treated surgically,while 168 of them prefered the conservative treatment.Methods The multivariate Logistic Regression with controlling the interactims of independent variables used to the survey to illustrate that,those who wewe younger,with low BMI,with course time longer,of better economic conditions,with rich experiences of treatment tended to surgical treatment to repair rotator cuff tear;on the contray,those who were elder,with high BMI,with course time shoter,of worse enonomic conditons,with little experience of treatment were found more easily to choose the expectant treatment;yet no significant condition was observed among gender,smoking history,with frozen shoulder or not and the United States shoulder elbow surgery score(ASES).Conclusion: Briefly,results indicates that [age,degree of education,economic level,body mass index(BMI),visit times,course time] have predictive value of approches of patients with rotator cuff tear,yet gender,smoking history,the extent of ratator cuff tear,with frozen shoulder or not and the United State shoulder elbow surgery score have little predictive value of them.In conclusion,when facing patients with ratator cuff tear,clinicians can judge the tendency of treatment approaches of patients by [age,degreeof education,economic level,body mass index(BMI),visit times,course time].once the mose appropriate therapy according to the patients real condition disagrees with their internal terdency,we clinicians are supposed to enhance communication with patients in time,explain the details of the disease particularly,elarify the state of illness and promote interactive information,thus we could lower costs,raise medical treatment effciency and alleviate the conflicts and medical disputes.
