| Objective:To evaluate the value of doppler parameters of uterine artery after swinging the cervix in the diagnosis of endometriosis.Methods:Before surgery, transabdominal ultrasound was performed. Each patient has been tested for doppler parameters(peak systolic velocity(PSV), end diastolic velocity(EDV),pulsatility index(PI), and resistance index(RI) of uterine artery before and after swinging the cervix.Results:54 cases were confirmed to be endometriosis by laparoscopy which is the gold standard of diagnosing endometriosis. Before and after swinging the cervix, area under the curve(AUC) of every parameter were PSV0.577/0.678,EDV0.792/0.884,RI0.850/0.918,and PI0.841/0.917. When the cut-off points are RI>0.85 and PI>1.47, the diagnostic efficiency of them are as follows :sensitivity 94.4%,specificity 77.2%,accuracy 88.3%,positive predictive value 77.2%, negative predictive value 86.6%, respectively. The positive rates of diagnosis are as follows: stage I 88.23%(15 / 17), stage II 92.31%(12 /13), stage III 94.12%(16 / 17), stage IV 100.00%(7 / 7). The diagnosis positive rate of cystic endometriosis is 100.00%(34 / 34) and non-cystic endometriosis is 80.00%(16/20).Conclusions:The doppler parameters of uterine artery before swinging the cervix have little value in the diagnosis of endometriosis,while RI and PI after swinging the cervix have high diagnostic value in the diagnosis of endometriosis, especially in early non-cystic endometriosis. |
