Objective: To research the risk factors of postoperative intracranial hematoma after craniotomy intracranial tumor resection.Methods: There were 1037 patients who accept craniotomy in proper time or within limit time in neurosurgery in Guangxi Medical University from January, 2013 to December, 2014. Based on comprehensive literature on the definition of intracranial hematoma after craniotomy[1-3], a total of 32 cases had postoperative intracranial hematoma. According to whether patients with intracranial hematoma after craniotomy intracranial tumor resection or not, they were divided into postoperative hematoma group and non postoperative hematoma group. Differences of patients’ general information(including gender and age), disease characteristics(including tumor types, tumor location, basic diseases, etc), and information related to the operation(including operation time, intraoperative blood loss, intraoperative intracranial external drainage) were compared between the two groups. The multi-factor Logistic regression analysis was calculated to determine the risk factors after screening the related factors of postoperative intracranial hematoma.Results: 1037 patients were selected in this research, 32 patients were in postoperative hematoma group, and 1005 patients were in non postoperative hematoma group. Significant differences(P<0.05) were found in tumor types, intraoperative blood loss, intraoperative intracranial external drainage and patients with hypertension or diabetes between the two groups after comparison. In Logistic multi-factors analysis model, the nature of the tumor(OR=5.876,95% CI: 1.118~2.868), intraoperative blood loss(OR=17.713,95% CI: 1.743~4.589), patients with hypertension(OR=8.792,95% CI: 1.623~10.722) is considered to be the independent risk factors of postoperative intracranial hematoma after craniotomy intracranial tumor resection.Conclusion: Many reasons can cause postoperative intracranial hematoma, including patients’ medical history, disease characteristics and various situations in the operation. In this research, the nature of the tumor, intraoperative blood loss, patients with hypertension is the independent risk factors of postoperative intracranial hematoma.
