Objective: To estimate the value of serum midkine in predicting of severity of acute pancreatits by observing the dynamic change of the serum midkine level in severe acute pancreatitis.Methods: 97 patients with acute pancreatitis from February,2014 to February,2015 were included in the study. The serum midkine concentration was measured by ELISA kit on day 1,3,5,7 after admissionand the day before discharge. According to the severity of thedisease, the occurrence of pancreatic necrosis and the occurrence of organfailure, patients were divided into three groups separately. Average serum midkine values were compared between groups by t test forunpaired data. The difference was assumed statistically significant when p<0.05. A receiver operating characteristic(ROC) curve analysis was used to explore the relationship between the sensitivity and specificity of prognostic indices in determining disease severity.Results: The serum midkine concentrations were significant elevated in all the patients with acute pancreatitis. The serum midkine concentrations were starting to decreased gradually as soon as theyreached the peak points on day3 after admission; Between patients with acute pancreatitis in mild group,mild severe group and severe group the serum midkine concentrations were decreased gradually and the difference was statistically significant(p<0.05).The serum midkine concentrations of pancreaticnecrosis group were higer than those of non pancreaticnecrosis group, he difference was statistically significant(p<0.05).According to the ROC curve, it is suggests that MK can predict the severity andsystemic of acute pancreatitis.Conclusions: The results of our study show that the serum midkine level was increased in acute pancreatitis and it can be used as an early important predictive marker of severity of acute pancreatits.
