Objectives:Through testing the levels of serum PTX-3at early stage, to debate its value in early prediction of severe acute pancreatitis. Studying its correlations with other diagnostic methods, in addition to compare their diagnostic capacity.Methods:From October2013to February2014, thirty seven patients diagnosed with acute pancreatitis were analyzed in this study According to Chinese guideline of acute pancreatitis treatment and diagnosis, patients are classified into two groups as mild acute pancreatitis group(20cases) and severe pancreatits group(17cases). Peripheral blood was taken on the1d,4d,7d to detect PTX-3. In the first24h of admission, CRP and PCT were measured. APACHEII, CTSI and BISAP were also calculated on the first day.Results:(1) In the first day after admission, the concentrations of PTX-3in SAP group and MAP group were higher than control group(P<0.001), and PTX-3levels in SAP group were also higher than MAP group(P<0.05); The concentrations of PTX-3within7days at the same point after admission were higher in SAP group than in MAP group(P<0.05); At the best cuf-off level of11.30ng/ml, diagnostic sensitivity for SAP is66.7%and specificity is78,3%.(2) Expression of PTX-3is negatively related to CRP expression (r=0.312, P=0.068) and positively related to PCT expression(r=0.582, P<0.01)(3) Expression of PTX-3had positive correlations with APACHEII and BISAP(P<0.05), and had negative correlations with CTSI(r=0.201, P=0.248). (4) Receiver operating characteristic cure showed that APACHEII(AUC=0.975) had the best diagnostic capacity, followed by BISAP(AUC=0.855), CTSI(AUC=0.848), PCT (AUC=0.841)and PTX-3(AUC=0.780).Conclusion:(1) The serum PTX-3is closely related to the severity of acute pancreatitis, and it could be a new biomarker in early prediction of SAP. At the cut-off value of11.30ng/ml, PTX-3has the best diagnostic capacity for SAP.(2) At the early stage of acute pancreatitis, PTX-3is involved in and positive related to the inflammatory process.
