Positive Reacted Point In The Auricular Acupressure As A Preventive Treatment For Gynecological Late Cancer TP/TC Chemotherapy-induced Nausea And Vomiting

ObjectiveBy researching the clinical effect of positive reacted point in the auricular acupressure as a preventive treatment for gynecological late cancer TP/TC chemotherapy-induced nausea and vomiting and exploring its security and feasibility, We hope to find a kind of traditional Chinese medicine treatment method which is effective、 little side effect、 easy to clinical implement and to accept for the patient.MethodsThe study was a prospective single-blind, randomized controlled trial. 78 patients gathered were randomly divided into three groups:positive reacted point in the auricular acupressure and western medicine group, non-positive reacted point in the auricular acupressure and western medicine group, western medicine group, each group of 26 cases. This testing program design was that patients were given randomly to intravenous ondansetron 8 mg plus 10 mg saline or add with positive reacted point in the auricular acupressure or add with non-positive reacted point in the auricular acupressure before chemotherapy. Through this clinical trial we observed the occurrence of nausea and vomiting, intake and output and the emergence of other side reactions on the first to third day of chemotherapy, compared scores of questionnaire of QOL and symptoms of Chinese medicine before and after chemotherapy. The test date were evaluated by SPSS.ResultsThis study was to aspects of chemotherapy-related nausea and vomiting treatment and explore the clinical effect of positive reacted point in the auricular acupressure as a preventive treatment for gynecological late cancer TP/TC chemotherapy-induced nausea and vomiting, the results were as follows:(Note:A is positive reacted point in the auricular acupressure and western medicine group; B is non-positive reacted point in the auricular acupressure and western medicine group; C is western medicine group).(一) Evaluating from the treatment effect of nausea and vomiting on the first to third day of chemotherapy1. Effective rate of nauseaEfficient of group A、 group B、 group C was 100%、100%、 96.2% respectively on the first day, there was not statistically significant comparison among the three groups (P>0.05). On the second to third day, effective rate of three groups was 88.5% 80.8%、 38.5%respectively, There was significant differences between group A and group C, group B and group C(P＜0.05)but did not find between group A and group B (P>0.05), which showed the efficacy of group A, group B were significantly better than that of group C, but no obvious difference between A and B.2. Curative efficiency of vomitingOn the first day, effective percentage of group A、 group B、 group C was88.5% 84.6%、 76.9% respectively, there was no statistical significance among the three groups. On the second to third day, respond rate of three groups was 73.1% 57.7%、 30.8%, the difference between group A and group C was statistically significant (P＜0.05). There were no visible difference between group A and group B、 group B and group C (P>0.05). Those indicated that efficient of group A was obviously higher than that of group C, no obvious advantage existed between group A and group B、 group B and group C.3. Analysis of vomiting frequencyThere were not statistically significant comparison among the three groups on the first day(P>0.05). On the second to third day, no significant differences showed between group A and group B (P>0.05), which announced that there were no significantly superior of therapeutic efficacy between the two groups. Contrast between group A and group C, group B and group C were notable difference(P＜0.05), which indicates that, on the second to three day, the curative effect of group A and group B are superior to group C.(二) Per capita food intakeThe first day of comparison among the three groups had no statistical significance (P>0.05). On the second to three day, Group A compared with group B had no statistical significance(P>0.05); Compared with group C, group A and group B had significant difference (p＜0.05); Those showed that compared with group C, group A and group B patients could significantly improve the appetite of the second and third day, but the effect of group A and group B was no significant difference.(三) Plus anti-emeticsFrom the first to the third day of chemotherapy, comparison between group A and group C was statistically significant (P<.0.05) but not between group A and group B, group B and group C (P>0.05). This indicates that group A could reduce obviously the dosage of additional antiemetic during chemotherapy but did not group B.(四) Stool aspectFrom the total number of cases of constipation, there was not statistically significant between any two groups (P>0.05). This showed in group A and group B were not obviously improve the symptoms of patients with constipation.(五) the quality of life, clinical symptoms and bone marrow suppressionKPS score:there was no obvious difference between the three groups before and after chemotherapy (P>0.05). EORTC-QLQ and TCM symptoms score:score of the three groups patients was significant before and after chemotherapy (p＜0.05). But it was no obvious distinguish(P>0.05)after chemotherapy and the difference between before and after chemotherapy among three groups as well as bone marrow suppression after chemotherapy.(六) Biochemical index testsAfter chemotherapy and the difference between before and after chemotherapy, there existed no obvious distinguish among three groups(P>0.05).ConclusionThe clinical research proof:Combination of ondansetron,1. positive and non-positive points ear pressure, on the second to third day, could significantly reduce the degree of nausea and the number of vomiting as well as improve patient’s appetite.positive points ear pressure could significantly improve the efficacy of delayed vomiting, but positive points ear pressure showed no significant efficacy advantage compared with non-positive points ear pressure.2. On the first to third day, positive points ear pressure could significantly reduce the amount of antiemetic drugs but could not non-positive3. On the first to third day, the positive and non-positive ear pressure bean is not significantly improve patient’s constipation and quality of life、 relieve TCM symptoms and improve bone marrow suppression caused by chemotherapy.4. Ear pressure bean is safe and no obvious side effects.
