Objective Based on Modified early warning score (Modified early warning score, MEWS) and Standardized Modified early warning score system for clinical application comparison and evaluation, to verify the correction of MEWS application value in patients with acute cerebral apoplexy.MethodsFrom April 2015 to March 2016, take the convenience sampling method to select one of top three hospital neurology ward in Shandong province.326 cases of acute cerebral apoplexy patients as the research object. Medical staff admitted to the ward for the first time, respectively, were modified early warning score (mews) and correction of mews, resulting in the initiation of the appropriate level of medical interventions, to 30 days after the end of the patients for the outcome.Results1.The research object into acute cerebral stroke patients in 326 cases, male186 cases, female 140 cases; age ranged from 48 to 78 years old, the average (63.2±8.9) years; the actual improvement or recovery into a group of 295 cases,25 cases in ICU group,6 cases in death group。2. All the cases were divided into three groups:<50,50-70,>70 and each group were compared.<50 years old patients the prognosis or improved in 54 cases, to ICU2 cases,0 cases of death;the prognosis of 50±70 or better in 178 cases, to ICU8 cases,2 cases of death; the prognosis of>70 or better in 63 cases, to ICU 15 cases,4 cases of death.3.There was no statistically significant difference in two case, cured or improved between Standardized modified early warning score and MEWS score of (P>0.05). The improvement or recovery of patients with MEWS and Standardized modified early warning score were compared, the difference was statistically significant (P<0.05); group MEWS patients into ICU and Standardized MEWS score distribution, the difference was statistically significant (P<0.05); death group of patients with MEWS and Standardized modified early warning score distribution, the difference was statistically significant (P<0.05); improvement or cure into the group, ICU group and death group Standardized modified early warning score were higher than the MEWS score, the difference was statistically significant (P<0.05).4. The ability to identify whether to death or ICU, ROC curve area of the two were 0.989 and 0.770. Correction of MEWS score was significantly higher than that of MEWS score (U=9.95, P<0.01). MEWS and MEWS score ROC curve correction of acute stroke patients into ICU and the optimal cut-off value of MEWS score:ROC area under the curve (0.770+0.067) (95%:0.848-0.908, P=0.000) modified MEWS score of 1.5 points, sensitivity:86.7% specificity:51.4%, Youden refers to the maximum number of calibration area; MEWS score under the ROC curve (0.989+ 0.006) (95%:0.884-0.930, P=0.000) adjusted MEWS scores of 3.5, sensitivity:100% specificity:98.4%, the largest Youden exponent. MEWS and MEWS were used in the prediction correction of acute stroke patients were transferred to ICU, the difference was statistically significant (P=0.000), two area under the curve of ROC score difference was statistically significant (P<0.05).Conclusions1. The incidence, severity and death rate of the acute cerebral stroke patients of 50 to 70 years of age were higher than those whose age below 50 or more than 70 years. So a high degree of vigilance should be emphasized to the patents of 50 to 70 years in the process of clinical nursing activities.2. The Standardized MEWS score and the MEWS score could predict early severity of acute cerebral stroke patients, and the operation is simple and accurate, it is worth to promote in the Department of internal medicine ward.3. Compared with MEWS, the Standardized MEWS was more than MEWS score to predict the severity, transferred to ICU or death of acute cerebral stroke patients the Department of internal medicine ward.
