Objective:This study aims to develop a premature infant disease observation program based on the modified neonatal early warning score,and explore the scheme applied to the roles of premature clinical nursing observation,aims to help clinical nurses found early signs of premature condition change,and take the necessary medical interventions,raise children recovered discharge rate,reduce the children hospitalization days and the hospitalization expenses,and promote the doctor for a nurse observation ability degree of satisfaction.Methods:1.Based on the literature analysis method,integrate the domestic and foreign intervention plans using early warning scores,analyze from the level of nursing management and nursing assessment,and combine environmental and patient factors to develop a preliminary disease observation plan based on the improved newborn early warning score system.Then the expert group discussion method was adopted to develop the final plan.2.The cluster sampling method was used to select 220 premature babies treated in a tertiary first-class hospital from April to November 2019 as the research object,and then to take 110 premature babies treated from April to May 2019 as the control group.The conventional observation program for the condition,taking 110 preterm infants admitted in October-November 2019 as the observation group,implemented a disease observation program based on the improved neonatal early warning scoring system,and compared the number of medical interventions,recovery rates,Differences in hospitalization days,hospitalization costs,and doctors 'satisfaction with nurses' ability to observe conditions.Results:1.The number of medical interventions in the observation group was 13.42±11.2times,higher than that in the control group 9.86±8.70 times,with statistically significant difference(P<0.05).The number of medical interventions in theobservation group,such as clearing the respiratory tract,upgrading antibiotics was higher than that in the control group(P < 0.05),and the difference was statistically significant.The number of cardiopulmonary resuscitation was lower than that of the control group(P < 0.05),and the difference was statistically significant.Compared with the control group,the number of medical interventions such as capacity expansion,acid-base balance correction,blood transfusion,cooling/rewarming and so on was not statistically significant(P > 0.05).The recovery and discharge rate of the observation group was higher than that of the control group,and the difference was statistically significant(P < 0.05).The recovery and discharg rate of the observation group was higher than that of the control group,and the difference was statistically significant.2.The length of hospitalization in the observation group was 12.49±10.33 days,and the hospitalization cost was 1.88±1.92 yuan,which was lower than the17.81±15.12 days and 2.70±2.76 yuan in the control group and the hospitalization cost,with statistically significant differences(P < 0.05).3.Doctors' satisfaction with nurses' observation ability was higher than that of the control group,and the difference was statistically significant(P < 0.05).Conclusion:1.The disease observation scheme based on the neonatal early warning score is relatively practical and reliable,which can help clinical nurses to find signs of changes in premature infants' condition at an early stage,and take necessary medical intervention measures to premature infants at an early stage,so as to improve the rate of premature infants' recovery and discharge.2.Early detection of signs of changes in premature infants' condition and effective intervention measures are conducive to reducing the hospitalization days and hospitalization costs of premature infants.3.Nurses have the ability to detect the observation signs of premature infants at an early stage,which helps to improve doctors' satisfaction with nurses' observation ability and promote effective communication between doctors and nurses.
