Development And Evaluation Of The Quality Of Life Instrument For Caneer Patients-prostate Caneer(QLICP-PR)

Objective:To develop and validate the quality of life instrument for caneer patients-prostate cancer(QLICP-PR)and the Chinese version of EORTC QLQ-PR55,and to compare both of them.Methods:Based on the modular approach(a general modules plus specific modules)and Chinese culture, and also the programmed decision procedure including nominal group and focus group diseussion,in-depth interview,pilot tests,pre-tests and the statistical proeedures for items eleetion,a specific QOL instrument for patients with prostate cancer QLICP-PR has been developed. A Chinese version of the EORTC QLQ-PR55 was developed by strict translation procedures of EORTC translation guidelines,including forward translation,baekward translation,culture adaptation,and pilot-testing.Two instoments(QLQ-PR55,QLICP-PR)were used to a sample of 115 patients with prostate cancer at the time of their admission to the hospital,a sub-sample of patients(n=94)was sampled and assessed a second time 1-2 days after hospitalization;a sub-sample of patients(n=99)was sampled and measured a third time at discharge,so that to evaluate validity,reliability and responsiveness of two instruments.Two instruments were compared with respect to contents and psychometries.The statistic tools used in this study inelude statistic deseription、 regression、t-test、 exploratory factor analysis and so on.Results:1.21 items of prostate cancer specific module was developed,combining with the existing 32 items of the general module QLICP-GM to constitute the QLICP-PR.And the Chinese version of EORTC QLQ-PR55 was also developed.2.Results on psychometrics of QLICP-PR showed test-retest-reliability of the seale is 0.920,Cronbaeh’s Alpha is 0.884, split-half reliability is 0.837.Generalizability theory research results showed that the item numbers of psychological domain(9)、 social domain(8)、common symptom and side effect domain(7)、specific module(1)were appropriate,but the item numbers of physical domain can be appropriately increased.Correlation and factor analysis showed general construct validity. The Chinese version of EORTC QLQ-PR55 as the criterion analysis shows that QLICP-PR scale has good criterion validity. Before and after treatment, in addition to sexual problems,and the paired t test of rest fields were statistically significant (P<0.05),showing the instrument can reflect QOL for patients with prostate cancer after treatment basicallly.3.The Chinese version of EORTC QLQ-PR55 was also evaluated and compared with QLICP-PR:The Chinese version of QLQ-PR55 and QLICP-PR both reflected the contents of the quality of life,and had differences in contents.Both instruments had good reliability、responsiveness, and general validity.Both instruments with reliability 、construct validity and responsiveness were similar,and had good content validity.Conelusion:QLICP-PR is of good validity,responsiveness,and general reliability,it can be used to measure QOL for Chinese patients with prostate cancer.The Chinese version of QLQ-PR55 and QLICP-PR have differences in contents,and psychometrics of two instruments are similar.
