Study On The Cases Treated From Phlegm-stasis Of Senile Disease By Professor Zhou Zhongying

[Objective]Based on the research of Chinese medicine master Zhou Zhongying’s academic thought of geriatric medicine by my tutor Jingqing Wang, by the way of collecting cases of professor Zhou of geriatric patients, to explore distribution of the syndrome of phlegm and blood stasis in the hypertension, cerebral infarction, coronary heart disease, Parkinson’s disease, diabetes and other common diseases of elderly, analyzing and comparing the therapeutic experience and prescription medication characteristics of the above-mentioned common geriatric diseases,to summary the dialectical thinking and rule of prescription of Professor Zhou,.deepening the understanding of the theory of traditional Chinese medicine on senile disease, to provide reference to improve the clinical diagnosis and treatment of senile disease and play the advantage of Chinese medicine in the treatment of senile diseases.[Method]By referring to the works by acient physicians and CNKI(China National Knowledge Infrastructure),carding the method of Chinese Medical treating senile diseases systematically, and learning about recent advances in Traditional Chinese medicine geriatrics,especially to grasp the base case of senile diseases treated from phlegm-stasis. According to inclusion criteria and exclusion criteria selecting cases treated from phegm-stasis by Professor Zhou during January 2010 and February 2015 strictly, and then according to the specification requirements cleaning and normativing data. With the 98 cases standard-compliant, we use Medcase V3.2 storehouse and medical record national physician pulse case data mining system software to establish a database. By the way of frequency distribution, strengthen association rules and clustering analysis mining method, to analyze the treatment rules of senile diseases of phlegm-stasis of Professor Zhou in the past five years, and to explore the relationship among the pathogenesis, clinical manifestations, prescription and drug rlues and so on. By the rule of "man-machine combination, to people based", to analyze the computer data mining results realistically.[Result](1) The highest frequency disease in the cases treated from phlegm-stasis by Professor Zhou is cerebral infarction, frequency is 60 times, accounted for 61.22%, followed by hypertension, frequency of occurrence is 58 times, accounted for 59.18%, proportion of the two are more than half, both frequency greater than 1, reflecting that the majority of old people also had the characteristics of various diseases.(2) The selected medical records appear that the cumulative frequency is 263 times, much higher than the number of medical research, among 98 cases of medical record only 29 cases diagnosed by single disease, indicates that 70.41% of the cases of elderly patients have two or more than two kinds of diseases.(3) All of the clinical symptoms "dizziness" occurs 42 times, accounted for 42.86%, indicating that dizziness is a common symptom in elderly patients. The cases occourence frequency of symptoms above 20%, according to the occurrence frequency, ordering from more to less is dizziness 42 times (42.86%), dry mouth 32 times (32.65%), constipation 32 times (32.65%), limb hemp 29 times (29.59%), adverse events 23 times (23.47%), chest tightness 21 times (21.43%).(4) After the specification, the recorded in the pulse slippery pulse of the highest frequency, a total of 72 times, far higher than other pulse of frequency of occurrence, accounting for 73.47% pulse records records all contained. After reading the original record observed pulse recording rules, find string slippery and smooth pulse is more common. The statistics of Taut Pulse and slippery pulse frequency was 41, fine and smooth pulse is 24 times, both the cumulative frequency of 65 times, accounting for the number of cases of 66.33%.(5) The record in the frequency is greater than the pathogenesis list 3 times, a total of 15. The frequency highest before five pathogenesis are liver and kidney deficiency, phlegm and blood stasis, deficiency of kidney and hyperactivity of liver-fire, endogenous wind dark, wind phlegm and blood stasis. Pathogenesis of record contains the virtual and the underlying real two aspects, which the virtual in liver and kidney deficit deficiency, deficiency of kidney and hyperactivity of liver-fire is the most common. And standard real terms include the syndrome of phlegm and blood stasis and appear at the same time, or can be summarized for kidney deficiency asthenia/ deficiency of kidney and hyperactivity of liver-fire+phlegm and blood stasis; the statistics of liver and kidney deficient virtual/deficiency of kidney and hyperactivity of liver-fire+phlegm and blood stasis in a total of 52 cases, accounted for the total number of cases of 53%; the insufficiency of liver and kidney deficiency and phlegm and blood stasis in 24 cases, accounted for the total number of cases of 24.49%, deficiency of kidney and hyperactivity of liver-fire+phlegm and blood stasis in 28 cases, accounts for the total cases number 28.57%.(6) Master Zhou from phlegm and blood stasis in treating senile disease medication up to 147 species, which of the frequency highest is Jiangcan,71 times (4.40%), the rest followed by 66 times of Tianma (4.09%) and Chuanxiong 59 times (3.65%), Danshen 53 times (3.28%), Shengdi 49 times (3.03%), Baijili 47 times (2.91%), Sangjisheng 43 times (2.66%),Jixueteng 43 times (2.66%).[Conclusion]1. Master Zhou nearly five years from phlegm and blood stasis in the treatment of senile disease mainly for cardiovascular and cerebrovascular diseases, the top three in turn is cerebral infarction, hypertension, coronary heart disease.2. Master Zhou from phlegm and blood stasis in the treatment of elderly patients with so many symptoms,but the analysis of its characteristics and phlegm stasis obstruction of the location of the relevant. Such as phlegm and blood stasis in brain can see dizziness; In meridian, limb numbness, or even in heart vessel can see chest stufly.3. Master Zhou from phlegm and blood stasis in the treatment of elderly patients’Pulse condition mostly slippery pulse,thready-slippery pulse and Stringy-slippery pulse more common.4. From case analysis, phlegm and blood stasis is enrichment in symptom partthere is still deficiency in origin. Common "deficiency of the liver and kidney+phlegm and blood stasis", "deficiency of kidney and hyperactivity of liver-fire+phlegm and blood stasis" in two forms, the main treatment method for nourishing liver and kidney, eliminating phlegm and removing blood stasis.5. The most commonly used medicine to feliminating phlegm is Jiangcan, Tiannanxing, Gualou, Shichangpu; use the highest frequency of removing blood stasis drugs followed by Chuanxiong, Danahen, Jixueteng, Shuizhi, Taoren; use the highest frequency of tonifying liver and kidney drugs followed byShengdi, Sangjisheng, Maidong, Shihu, Heshouwu; frequency highest calming liver wind drugs followed by Tianma, Baijili, Xiakucao, Gouteng, Juhua, Luobumaye.
