Objective1. To compare the difference in the clinical efficacy between Shu Mu acupoints acupuncture and conventional acupuncture on the treatment of yin-deficiency and fire-hyperactivity type of post-stroke insomnia on the basis of conventional treatment.2. To observe the effect of Shu Mu acupoints acupuncture on the sleep quality and the activities of daily living in post-stroke and discuss its functional mechanism.Methods70 cases of yin-deficiency and fire-hyperactivity type of post-stroke insomnia were randomly divided into an test group and a control group,35 cases each. On the basis of treatment of underlying disease, brain nutrition, prevention of complications and nutritional support, the control group received the acupuncture therapy with conventional points(such as Shenmen, Neiguan, Baihui, Anmian, Taixi, Taichong and Yongquan) and the test group received the acupuncture therapy with Shu Mu acupoints (such as Xinshu, Juque, Shenshu, Jingmen, Ganshu and Qimen). The treatment was given once a day and lasted for two courses (6 days for a course, at the end of a course, take a rest for one day) in both group. To evaluate the total syndrome score, Pittsburgh Sleep Quality Index(PSQI), Self-Rating Scale of Sleep(SRSS) and Barthel index(BI) of patients in two groups respectively before and after treatment and the clinical therapeutic effects were compared between the two groups.Results1. General informationBefore treatment, two groups of patients at the aspects of factors(such as gender, age, course of disease and underlying disease) were no significant difference (P>0.05), so the source of two groups could be consistent and comparable.2. The results of the total syndrome scoreAfter treatment, the total syndrome scores were improved in two groups, the differences before and after the treatment had statistically significant (P <0.05). Compared between two groups, the improvement of scores in the test group was better than the control group after treatment, the difference of them had statistically significant (P<0.05). It means that the test group is better than the control group at the improvement of the total syndrome score.3. The results of PSQIAfter treatment, the PSQI scores were improved in two groups, the differences before and after the treatment had statistically significant(P <0.05). Compared between two groups, the improvement of scores in the test group was better than the control group after treatment, the difference of them had statistically significant (P<0.05). It means that the test group is better than the control group at the improvement of the PSQI score.4. The results of SRSSAfter treatment, the SRSS scores were improved in two groups, the differences before and after the treatment had statistically significant(P <0.05). Compared between two groups, the improvement of scores in the test group was better than the control group after treatment, the difference of them had statistically significant (P<0.05). It means that the test group is better than the control group at the improvement of the SRSS score.5. The results of BIAfter treatment, the BI scores were improved in two groups, the differences before and after the treatment had statistically significant(P< 0.05). Compared between two groups, the improvement of scores in the test group was better than the control group after treatment, the difference of them had statistically significant(P<0.05). It means that the test group is better than the control group at the improvement of the BI score.6. The results of the clinical therapeuticAfter treatment, in the test group,5 cases were cured basically,17 casts were remarkably improved,10 cases were effective,3 cases were ineffective and the total effective rate was 91.4%(32/35); in the control group,2 cases were cured basically,11 casts were remarkably improved,16 cases were effective,6 cases were ineffective and the total effective rate was 82.9%(29/35). The difference between two groups had statistic significance(P <0.05). It means that the test group is better than the control group at the aspect of the clinical therapeutic.Conclusion1. Compared with conventional acupuncture, Shu Mu acupoints acupuncture is more efficiently on the improvement of clinical symptom in the patients of yin-deficiency and fire-hyperactivity type of post-stroke insomnia. The clinical efficacy of Shu Mu acupoints acupuncture is better than that of conventional acupuncture.2. Shu Mu acupoints acupuncture can observably improve the sleep quality after stroke and has a positive impact on the improvement of activities of daily living of patients. Its functional mechanism is likely to the recovery of Yuanshen, the balance of yin and yang and the storing of Zangshen.
