BackgroundKnee Osteoarthritis (KOA) is defined as a kind of degenerative disease, the patient often suffers from aching、stiffing、swelling besides with difficulty in movements such as walk up/down stairs, crouch down stand up etc as well as susceptible to pain or fatigue in work and daily activities, their life quality therefore can be influenced in different degree, defects and disability may also happen because of the disease.Currently there are various clinical therapy for KOA treatment. Non-operative therapy is mainly focus on improving life quality of patients by relieve the clinical symptoms and improving the joint function of patients. Studying in more convenient and effective therapies has great clinical significance in KOA treatment. Acupuncture treatment, a simple and high safety therapy has remarkable effects in curing KOA, the four points around knee joint are commonly used acupoints of the acupuncture treatment for KOA. Using penetration needling method can strengthen the needling sensation and the stimulation of the acupoints thus to get better curative effect.objectiveTo discuss simple and accurate clinical therapy basing literature review, in which epidemiological studies、Pathogenesis of both modern medicine and traditional Chinese Medicine, as well as clinical therapy for KOA have been inducted and reorganized. This study take acupuncture as mainly treatment therapy, take the frequently used four points around knee joint, using penetration needling method combines with Electro acupuncture to find more effective acupuncture treatment plans through observing patients symptom modification situation before and after taken treatment, and compare with common acupuncture therapy.Methods1. Literature reviewReview and summarize theoretical research results in aspects such as definition of KOA, epidemiological studies, Pathogenesis of both modern medicine and traditional Chinese Medicine and common clinical treatment therapy in recent years.2. Clinical researchThe research object of this study were selected from the first affiliated hospital of Guangzhou university of Chinese medicine acupuncture clinic patients, totally 60 cases with knee osteoarthritis, were randomly divided into treatment group (30 cases) and control group (30 cases). For treatment group, chose the four points around knee joint (Yang Ling Quan, Yin Ling Quan, Liang Qiu, Xue Hai), using penetration needling method combines with Electro acupuncture, take treatment for 30 minutes every two days, five times for a course, and 2 courses in total. For control group, take Liang Qiu, Xi Yan, Yang Ling Quan, and Xi Yang Guan four points, using Electro acupuncture, take treatment for 30 minutes every two days, five times for a course, and 2 courses in total. Observe symptom modification situation before and after treatment courses, taking the McGill pain scale (MPQ) and the Lysholm knee scoring criteria as observation indexes to measure the curative effect difference between treatment group and control group.Results:1.30 treatment group samples, among them male 12, female 18, aged between 41-80,30 matched group samples, within which male 13, female 17, aged between 42-77. There are no obviously difference in gender and age aspects in sample groups, comparable.2. There are no obviously difference in MPQ and Lysholm scores in sample groups before treatment, comparable.3. There are significant difference in MPQ score before and after treatment in both groups. Compare the variation of MPQ score between two groups, there is statistical significance. Treatment group and control group can significantly reduce the MPQ score, and the treatment group shows better effect in reducing MPQ score than control group.4. There are significant difference in Lysholm score before and after treatment in both groups. Compare the variation of Lysholm score between two groups, there is statistical significance. Treatment group and control group can significantly reduce the Lysholm score, and the treatment group shows better effect in reducing Lysholm score than control group.5. After treatment, there were 7 cases cured,19 cases were effective, and 4 cases had no obviously effect in treatment group, the total effective rate was 86.67%; In control group there were 3 cases cured,17 cases were effective, and 10 cases had no obviously effect, the total effective rate was 66.67%. The Ridit analysis shows that the curative effect of treatment group was obviously better than the control group.Conclusion:Observation results show that acupuncture treatment has good effect for treating knee osteoarthritis. The therapy in treatment group and control group can both obviously relieve knee joint aching, improve knee joint function. The total effective rate of treatment group was 86.67%, and in control group the total effective rate was 66.67%. In treatment group, using penetration needling method combines with Electro acupuncture at the four points around knee joint improve the function of knee joint and its curative effect is better than that of control group which uses common acupuncture treatment. This observation result can be used as a clinical reference for acupuncture treatment of knee osteoarthritis.
