Effects Of Preoperative Feeding With A Whey Protein Plus Carbohydrate Drink Before Hip Fracture Operation Of Elder: A Randomized Trial

Objective:This study focused on the effects of oral administration of whey protein on blood glucose, C- reactive protein, serum albumin, and length of stay after surgery on elderly patients with hip fracture. Methods:A total of 40 elderly patients with hip fracture were included in this study. The patients were grouped into two randomly:carbohydrate-protein group(PR) and the control group(CO). PR group received a solution containing 14% whey protein and 86% carbohydrates 400 ml in the night before operation and 200 ml at 3 hours before operation. The CO group received the same volume of water at the same time as PR group.Observation items: Ultrasound results of the size of the liquid dark area in gastric antrum(D1×D2); Bleeding volume during operation; operation time; Nausea and vomiting thirst and hunger after surgery; Laboratory examination results of blood glucose, CRP, albumin on the day before surgery and the 24 h after surgery; And the length of stay after surgery. Results:1. Preoperative feeding with a whey protein plus carbohydrate drink before hip fracture operation of elder did not increase the area of gastric antrum under ultrasound.2. Postoperative nausea and vomiting were not statistically different(P > 0.05). The thirst and hunger feeling of patient after the operation in PR group were less than CO group(P < 0.05).3. The difference of blood glucose before and after operation was greater in CO group(P < 0.05). Serum albumin was higher in PR group than in CO group(P < 0.05) after surgery. PR group was higher in CRP(P < 0.05) as well as the length of stay(P < 0.05) after surgery. Conclusion:Oral administration of carbohydrate solution containing whey protein before hip fracture operation of elder does not increase the amount of gastric contents. And it reduced the postoperative thirst and hunger, and resulted a higher level of albumin after surgery and reduced the increase of CRP. The length of stay after surgery were also reduced.
