Objective To investigate the prevalence and characteristics of thyroid nodule and its relationship with thyroid function in physical examination population in Xining area.METHODS A retrospective analysis of the survey was performed in the subject who were on routine physical examination in Qinghai Province Hospital. All subjects underwent ultrasound thyroid scan, and the prevalence of thyroid nodule and its quantity, property and distribution were compared by different gender, age and BMI groups. We analysed the clinical characteristics of thyroid nodules in physical examination population in xining area.The relationship between thyroid nodule existence and thyroid function was assessed in subjects who randomly accepted thyroid function test. We adopted the method of multi-factor Logistic regression and analyzed the independent risk factors associated with thyroid nodules occurred.RESULTS(1) The study included 1673 cases, the age distribution is between 21 to 96 years, mean age 49.64 ± 11.45 years; male 1227 cases(73.3%), mean age 49.66 ± 11.54(21 ~ 96 years), female 446 cases(26.7%), mean age 49.59 ± 11.20 years(22 to 83 years).(2) In this study, 644 cases of thyroid nodules were detected, the detection rate was 38.5%. In 644 cases of patients with thyroid nodules, solitary nodule 327 cases accounted for 50.8%, accounting for 317 cases of multiple nodules 49.2%.Solid nodules 315 cases, accounting for 55.5%, cystic 124 cases, accounting for 19.3%, cystic-solid 162 cases, accounting for 25.2%.(3) The study showed that the overall detection rate was 38.5%. Males were detected in 416 cases of thyroid nodules, the detection rate was 33.9%.Females were found in 228 cases of thyroid nodules, the detection rate was 51.1%.The detection rate of female is higher than male.There was statistical difference between the two groups(?2=40.95,P<0.001). Thyroid nodule detection rate in all populations, and different genders, gradually increased with age, the chi-square test showed statistically significant(P<0.001),but no correlation with BMI(?2=5.039,P=0.077).(4) The number of thyroid nodules aspect,the proportion of multiple nodules slightly higher in women than men, there was significant differences between the two groups(?2=10.62,P=0.001). With the age gradually increased the proportion of multiple nodules increased(?2=29.69,P<0.001),and no correlation with different BMI groups(?2=1.163,P=0.762).(5) In the composition ratio of thyroid nodules aspect, thyroid solid nodules has nothing to do with the age group,but the cystic-solid nodules increased with increasing age, cystic nodules decreased with increasing age(?2=34.84,P=0.006). There was no difference between genders and different BMI groups(P=0.455,P=0.549).(6) The study included 1673 cases, 1353 patients underwent thyroid function reached 80.9%. According to the TSH level: TSH<0.05 u IU/Ml 18cases(1.3%),TSH 0.05~IU/m L 1115cases(82.4%), TSH 4.78 u IU/m L 183cases(13.5%), TSH≥10.0 u IU/m L 37 cases(2.7%). In this study, thyroid function in the normal range up to 82.4%, subclinical hyperthyroidism 1.3%, subclinical hypothyroidism(13.5%), hypothyroidism 2.7%.(7) 1353 cases received thyroid function test, in accordance with thyroid ultrasound results are divided into groups without nodules and nodular group. The results showed that FT3,FT4 levels of thyroid nodules group than those without nodules decreased statistically(P=0.014 and P=0.001), the difference of TSH,TPOAb levels between the two groups was not statistically significant(P=0.054 and P=0.931).(8) Adopt the method of Logistic regression analysis showed that age, gender, BMI, total cholesterol(TC), triglyceride(TG), uric acid(UA), fasting plasma glucose(FG) is the independent risk factors associated with thyroid nodules occurred(P < 0.001, P < 0.001, P = 0.019, P = 0.001, P < 0.001, P < 0.001, P = 0.008), and thyroid function(FT3, FT4, TSH) have no obvious correlation.CONCLUSION(1) Thyroid nodule detection rate is 38.5% by ultrasound examination in physicl examination population in Xining area. The detection rate of female is higher than male,and increased gradually with age. Single nodule, solid nodules based.(2) The number of thyroid nodules aspect, the multiple nodules proportion of women was slightly higher than men and increased gradually with age.(3) In the composition ratio of thyroid nodules aspect. There was no correlation between thyroid solid nodules proportion with age.But the cystic-solid nodules proportion increased with age, cystic nodules decreased with age(?2=34.84,P=0.006).(4) Age, gender, BMI, total cholesterol(TC), triglyceride(TG), uric acid(UA), fasting plasma glucose(FG) is the independent risk factors associated with thyroid nodules occurred.
