Objective Through developing the scale on home care demands of fracture patients, to understand the current situation of the home care demands of fracture patients,to analyze the differences of different demographic characteristics on patients’home care demands and to identify the influence factors on the situation of patients’home care demands, the development of this studytends to lay a solid foundation for the further study of home care demands and provides the reference basis forformulating the home care services.Methods Test the reliability and validity of this scale. This study belongs to the descriptive study in non-experimental study. In accordance with the convenience sampling method, five comprehensive hospitalsin Yanbian area have been chosen as the sampling unit; 412questionnaires have been issued with 398 valid questionnaires returned, so the valid return rate is 96.6%. The research toolsinclude general demographic data, the self-designed questionnaire for home care demands of fracture patients. The statistical soft wares such as EpiData, SPSS 17.0, AMOS 17.0 are used for data input, data transformation, data sorting and analysis. The methods used in this study are percentage, mean, standard deviation, t-test, F-test, Pearson correlation analysis, multiple regression analysis, factor analysis and confirmatory analysis.Results1. The reliability and validity of the scale on home care demands of fracture patients. Project analysis results show that there are 5 dimensions and 32 items in the scale on home care demands of fracture patients. Reliability:(1) In the scale, the overall Cronbach’s alpha coefficient is 0.948, the Cronbach’s alpha coefficient of the mental demandsdimensionis 0.768, the Cronbach’s alpha coefficient of the life care demands dimension is 0.831, the Cronbach’s alpha coefficient of the health guidance demands dimension is 0.842, the Cronbach’s alpha coefficient of the safety care demands and rehabilitation care demands dimension are 0.818 and 0.806 respectively. (2) The split-half reliability of this scale is 0.910, and each dimension’s split-half reliability alpha is over 0.75. Validity:(1) Content Validity:Expert evaluation results show that S-CVI of the scale is 0.948, I-CVI level in each item is 0.67-1.00. (2) Construct Validity: Five factor models are include in exploratory factor analysis, and the cumulative contribution rate is 59.181%; Confirmatory factor analysis results show that the model of index chi-square=579.585, chi-square/df=1.554, the overall standardized hypothesis model residual RMR =0.008, the average likelihood RMSEA=0.037, TLI=0.962, NFI=0.924, CFI=0.971, GFI=0.920, which indicate the basic adaptation index reach the standard of the model.2. The current situation influence factors analysis on home care demands of fracture patients(1) In the 398 investigated cases among the fracture patients, there are 363 patients having home care demands, making the overall rate 91.2% which is high. The average scoreof home care demands among the research objects is (0.50±0.31), the scores for home care demands from each dimension in turn is:health guidance demands (0.50±0.38), rehabilitation care demands (0.50±0.36), safety care demands (0.49±0.38), life care demands (0.45±0.36) and mental care demands (0.42±0.37)(2) There is no statistical difference (p>0.05) among different genders, different educational levels and different fracture parts about home care demands of patients. By the comparison of the average, it is found that male patients demand more than female patients in mental care, safety care and rehabilitation care; The higher the educational level the patients have, the more home care demands in every dimension they demand.(3) There are significant differences (p≤0.01) about the home care demands among the patients of different ages, nationalities, marital status, occupations, types of health care, places of residence, caregivers and their related knowledge of the disease. Among them, the Korean patients averagely demand more than the Han and other ethnic patients in mental care, life care, health guidance, safety care and rehabilitation care; the fracture patients who are over 60 years old, or live in towns alone, or know nothing about the related knowledge about the disease demand home care demands more.(4) There is statistical significance (p≤0.05) on home care demands of fracturepatients on the aspect of different monthly incomes and living ways. The demands of the patients whose monthly income between 3001-5000yuan are higher in four dimensions:the mental care, life care, health guidance and safety care than that of the other patients with different incomes; the demands of the patients living alone are higher in other four dimensions except the health guidance than other patients living in different ways.(5) The results of Logistics regression analysis show that the main factors thatinfluence the home care demands of fracture patients are occupations, living styles and the knowledge level about the disease.Conclusion1. The results of the designed questionnaire have good reliability and validity, which can be considered to be used to evaluate the home care demands of fracture patients.2. The home care demands rate of the fracture patients is 91.2%, and the overall demand rate is comparatively high.3. Factors influencing the home care demands of fracture patients are ages、nationalities、marital status、occupations、health care types、residential places、living styles、caregivers and the knowledge level about the disease.4. The main factors influencing the home care demands of fracture patients are occupations, living styles and the knowledge level about the disease.
