Hepatitis B Surface Antigen Levels In Hepatitis B Virus-Related HCC Patients Underwent Resection Of Hepatocellular Carcinoma Prognosis

Objective:To study survival preoperative and postoperative hepatitis B surface antigen level variety in Hepatitis B Virus-Related HCC patients underwent resection of hepatocellular carcinoma prognosis.Methods:From January 2008 to December 2013,consecutively 881 HBV-related HCC patients who underwent curative resection in Cancer Hospital of Guangxi Medical University (Nanning, China)were enrolled. Serum HBsAg levels were analyzed for association with HCC prognosis,together with other clinical variables.Results:Univariate analysis revealed that younger age, high levels of HBsAg, ALT>41U/L,AST>37U/L,TBIL>20.5μmol/L,AFP>20μg/L, ALB≤ 34 g/L,PLT≥100×109/L,multi nodularity, tumor size>5cm, Incomplete tumor capsule, BCLC B stage were found to be associated with HCC free survival. Not antiviral therapy,ALT>41U/L, AST>37U/L, AFP>20μg/L, ALB≤34 g/L, multi nodularity, tumor size> 5cm, Incomplete tumor capsule, BCLC B stage were found to be associated with HCC overall survival. Multivariate analyses showed that AST>37U/L, AFP>20μg/L, PLT>100×109/L, tumor size>5cm were independently associated with a increased risk of HCC free survival.Not antiviral therapy, high levels of HBsAg, AST> 37U/L, AFP> 20μg/L, albumin≤34 g/L, Incomplete tumor capsule, tumor size> 5cm were independently associated with a increased risk of HCC free survival. Compared with HCC patients with low levels of HBsAg,HCC patients with high levels of HBsAg were younger,and had a higher levels of ALT, AST, AFP. Multivariable stratified analyses showed HCC patients with low levels of HBsAg tended to have a better incidence of HCC free survival and overall survival in these subgroups of patients, including for not antiviral therapy, ALT<41U/L, single tumor,BCLC A stage.HCC patients with low levels of HBsAg had a better overall survival than HCC patients with high levels of HBsAg in antiviral therapy of patients (p=0.024). HCC patients with HBsAg level decline had a better overall survival than HCC patients with HBsAg level ascent in antiviral therapy of patients (p=0.009)Conclusions:High levels of HBsAg increase risk of HCC free survival and overall survival following curative resection. Postoperative antiviral therapy failed to improve HCC free survival, but thereby significantly prolonging overall survival. Critical value of HBsAg might as a important marker for evaluating whether patients received antiviral treatment after surgery. Postoperative high levels of HBsAg was found to be associated with HCC overall survival. HCC patients with postoperative HBsAg decline had a better overall survival.
