ObjectiveBy investigating clinical characteristics and pathology features of cryptogenic hepatocellular carcinoma(HCC) to improve the understanding of cryptogenic HCC.MethodsTwenty-two patients with cryptogenic HCC after hepatectomy in hepatobiliary surgery of our hospital were enrolled from January 2010 to December 2015, and the clinical characteristics and pathology features were retrospectively analyzed. The diagnostic criteriafor cryptogenic HCC were defined as HCC that was not related to hepatitis B and C,drug-induced liver injury, alcohol intakehistory, autoimmune hepatitis,Wilson’s disease and schistosomiasis. One hundred and thirty eight cases of HCC which related to hepatitis B and C were used as control group.The clinical features, imaging manifestations, biochemical results, tumor markers, as well as metabolic risk factors such asblood glucose abnormalities and hypertension were compared between cryptogenic HCC group and control group.ResultThe mean age of cryptogenic HCC group was older than the controlgroup(59.05±11.32 vs 53.42±10.15). The rate of female patients in cryptogenic HCC group was significantly higher than that in the control group(40.9% vs 20.29%). Compared with the control group, either the rate of hypertension history or the rate of blood glucose abnormalities or the rate of overweight in cryptogenic HCC group was significant higher(36.3% vs 17.4%,31.8% vs 13%, 40.9 % vs 18.8%).The rate of AFP≥400ng/dl in cryptogenic HCC group was significantly lower than that in the control group(13.64% vs 35.5%).Cholangiocarcinoma was most commonin cryptogenic HCC group. No significant differences in biochemical test and the diameter of neoplasm were been found in the two groups.Conclusion1 The mean age of cryptogenic HCC group was significantly older and the rate of female patients was higher.2 Cryptogenic HCC patients may have higher comorbidity with abnormal blood glucose and hypertension.3 Cryptogenic HCC patients may have higher comorbidity being overweight or obese.4 The level of AFP in cryptogenic HCC patients may lower than that of HCC related to hepatitis B and C.5 Cholangiocarcinoma was most common in cryptogenic HCC group.
