Objective:①To learn the Shanxi province primary hepatocellular carcinoma’s (PHC) clinical features in recent years②By exploring PHC’s relative etiological factors, to provide the scientific evidences for PHC’s effective prevention and control as well as to reduce the incidence with early diagnosis and treatment.Methods:Using retrospective analysis of many clinical datum of200PHC patients in our hospital,such as age, gender, job, initial symptoms, related factors, hepatitis B virus (HBV) markers,HBV genotype,tumor markers,antiviral treatment history,HBV mutants,and so on.Data entry and statistical analysis use SPSS13.0software.Enumerattion data use percentage, Measurement data use t test,comparison of the rate use χ2test inspection level a=0.05,and P<0.05has statistical significance.Results:①Within the200patients, Male/Female ratio(3.5:1), aged24~88(58.10±11.42) years, the paroxysmal age groupis50to60years which accounted for38%.Under the age of30years which accounted for1%were the least. Peasants were119cases of occupational structure which accounted for59%. With family history of cirrhosis was21%. With family history of primary hepatic carcinomas was6.5%.28%of patients with a history of alcohol consumption.25%of patients with a history of smoking.②No clinical specifities with the first symptom, which only appeared as abdominal discomfort, abdominal pain or swelling.There were also some with fewer symptoms,such as2cases were abnormal behavior and3cases were respiratory system behavior.③About79%of the PHC patients suffered from HBV infection, the Hepatitis virus infection rate was85.5%.64.5%of them with cirrhosis Simultaneously;④HBeAg-positive patients were less than the HBeAg-negative ones which were mainly characterized as "HBsAg (+), anti-HBe (+), anti-HBc (+)", accounting for51.3%(77/150); HBV DNA positive rate was64.0%(96/150), positive standard of HBV DNA was more than103IU/ml;⑤In150cases HBV-related primary hepatocellular carcinoma patients with60cases of genetic testing, C genotype patients accouted for80%; B genotype patients accouted for20%,did not find other genotypes.⑥Analysis of liver function damage:AST rised than ALT;ALP GGT rised significantly; increase of total bilirubin was no more than ALP GGT.⑦The positive rate of AFP in the PHC was72%, above400ng/ml was55%.Conclusion:PHC patients are mainly male, whose paroxysmal age groupis50to60years,there’s no clinical specifities with the first symptom,and most of them suffered from hepatic cirrosis, they are mainly characterized as "HBsAg (+), anti-HBe (+), anti-HBc (+)"and HBV-DNA load under105IU/ml. In the studying, HBV C genotype accounted for80%.There’s a close relationshiop between HBV C genotype and PHC.Smoking and drinking were bad for the PHC patients.Hepatic disfunction manly appeared as abnormal AST.The AFP leverl was worth of PHC’s diagnosis.
