Objective:The peri-implantitis is always considered to be the main cause of failure implant dentures, so people pay more and more attention to it.The purpose of the present study was to construct the peri-implantitis bone defect animal model, than study the osteogenic effect and compare with each other using various bone reconstruction method after completely basic treatment, to explore the best bone reconstruction method for peri-implantitis bone defect, to provide the experimental basis for the clinical work.Methods:The 6 healthy adult beagle dogs were used,then to extract bilateral two premolars.To choose 6 planting sites from 8 teeth sockets, immediate implant 6 DIO implants.To rapid establish peri-implantitis bone defect model through the “local bone + silk ligation + high glucose diet” comprehensive method after the successful osseointegration, then the 36 pieces implants with peri-implantitis were randomly divided into the blank control group, the traditional method group, the beta-TCP group, the BIO-GENE group, the Bio-Oss group and the non-curettage group, a total of 6 groups for the follow-up treatment.In addition to the blank control and non-curettage group(a total of 12 implants),all the implants were completely flap curettage, auxiliary semiconductor laser and periocline drug treatment until the soft tissue inflammation surrounding implant body disappeared(the bone defects still exist at the same time), Finally, to respectively perform bone reconstruction for the above 6 groups of implants suffering from peri-implantitis bone defect.3 months after the operation,To compare their differences using different bone reconstruction methods through direct measurement, X-ray examination, Micro-CT scanning, pathological section and new bone formation rate.Results:(1)The 36 implants showed the successful osseointegration in the 3 months after immediate implantation through the X-ray detection.(2)All of the implants with successful osseointegration were peri-implantitis and bone defect after 1 months of comprehensive method“local bone + silk ligation + high glucose diet”.(3)At the end of peri-implantitis bone defect model construction,the periodontal index and X-ray detection of all implants were consistent with each other,and there was no significant difference between groups(P>0.05).(4)The inflammation around the implants disappeared after the application of semiconductor laser and periocline,and there was a significant difference compared with the blank control and non-curettage group without completely basic treatment(P<0.05).(5)There was a significant difference among direct measurement index after the bone reconstruction(P<0.05),Bio-Oss group >BIO-GENE group >beta-TCP group(traditional therapy group) > control group(non-curettage group).(6)There was a significant difference among the D-value of bone defect height by X-ray(P<0.05),Bio-Oss group was better than BIO-GENE and beta-TCP group(traditional therapy group), control group(non-curettage group) was the worst.(7)There was a significant difference among the bone mineral density by Micro-CT scanning(P<0.05), the bone regeneration materials and autologous bone tissue has become almost the same in the Bio-Oss group,the high density image of bone material on the image was clearly visible in the BIO-GENE group,the bone material image was almost invisible within the region of interest in the beta-TCP group.(8)The histological sections showed that there was no new bone and still a large bone defect around the neck of the implant in the control and non-curettage group, there was a few new bone tissue and some osteoblasts in the traditional therapy and beta-TCP group,there was a large number of new bone cells in the Bio-Oss and BIO-GENE group.(9)There was a significant difference among the new bone formation rate(P<0.05),the NBFR was the highest in Bio-Oss group.Conclusion:After completely scaling,the comprehensive effect using “Bio-Oss + collagen membrane” is the most effective bone reconstruction method for peri-implantitis bone defect in the base of semiconductor laser and periocline drug treatment.
