Objective: To explore the instant effectiveness and safety of implanting a DES(drug eluting stent)in main vessel and dilating a small external diameter balloon in side branch in treating coronary bifurcation lesion.Methods: Forty eight patients who underwent this intervention strategy were retrospectively observed to explore its short-term effectiveness and safety.The reduction degree of the main vessel’s and side branch’s stenosis defined as the angiographic effectiveness was respectively evaluated after the operation.The occurrence of MACEs(major adverse cardiac events) including angina pectoris, acute myocardial infarction, revascularization and death during the operation was recorded as the clinical effectiveness.The pressure of main vessel sent balloon and side branch small balloon, the occurrence of branch occlusion, the occurrence of jailed branch small balloon and the occurrence of jailed branch guide wire.Results: The angiographic effectiveness is as follow:Before the operation the degree of main vessel’s stenosis was 86.80% ? 9.10%, while the degree of main vessel’s stenosis was0.20% ? 0.80% after the operation.The difference between them was statistically significative(z=-6.08,P<0.01).Before the operation the degree of side branch’s stenosis was 67.10% ? 20.00%, while the degree of side branch stenosis was 27.30% ? 18.20%after the operation.The difference between them was statistically significative( z=-5.990,P<0.01).The clinical effectiveness is as follow: no MACEs happened during the operation in this study.The safety is as follow: When the main vessel sent balloon was dilated in15.10 atm ? 1.80 atm and the side branch small balloon was dilated in 10.00 atm ? 2.70 atm,the jailed small balloon and guide-wire in the side branch could be removed smoothly.Conclusion: The instant angiographic and clinical effectiveness of implanting aDES in main vessel and dilating a small balloon in side branch in treating coronary bifurcation lesion is good.When the main vessel sent balloon is dilated in15.10 atm ? 1.80 atm and the side branch small balloon is dilated in 10.00 atm ? 2.70 atm,the safety of this PCI(Percutaneous Coronary Intervention)strategy is more well-pleasing.But its long-term effectiveness and safety is worth studying further.
