Characteristics Of Blood Pressure Variability Of Patients With Acute Posterior Circulation Infarction And Its Prognostic Value

Objective:To investigate the characteristics of blood pressure variability in patients with acute posterior circulation infarction, and to evaluate its prognostic value.Methods:Prospective analysis were made in patients with acute cerebral infarction in the department of neurology in Xinjiang People’s Hospital, 2015 March to 2016 January.The patients were divided into anterior circulation infarction group and posterior circulation infarction group according to the infarction location,using blood pressure variability(BPV) index by using24 h ambulatory blood pressure monitoring technology、collected the demographic data、risk factors of vascular disease、NIHSS score、blood pressure variability index of two groups at admission,and using single factor analysis and multi factor logistic regression model,to investigate the correlation between blood pressure variability and prognosis in patients with posterior circulation infarction.Results: There were 104 cases of anterior circulation infarction and 59 cases of posterior circulation infarction,logistic regression analysis showed that,more24hSBPSD(OR 1.657,95%CI 1.141-2.408,P=0.008)、24hSBPCV(OR 2.030,95%CI 1.180-3.493,P=0.011)、night SBPCV(OR 1.664,95%CI1.132-2.446,P=0.010)、night DBPSD(OR 2.086,95%CI 1.358-3.204,P=0.001)、non dipper(OR 8.444,95%CI 1.191-12.881,P=0.033)(P<0.05)in the posterior circulation infarction group were significantly higher than those in the anterior circulation infarction group.In patients with posterior circulation infarction,good prognosis group was 37cases(64.3%), poor prognosis group was 22 cases(35.7%),logistic regression analysis showed that 24hSBPCV(OR 2.821,95%CI 1.413-5.632,P=0.003)、night SBPCV(OR 1.591,95%CI 1.100-2.302,P=0.014)、night DBPSD(OR 2.191,95%CI 1.222-3.928,P=0.008)、non dipper(OR 6.660,95%CI 1.731-10.578,P=0.031)were associated with adverse outcomes in patients with posterior circulation infarction at threemonths.Conclusion:The blood pressure variability in the posterior circulation infarction group was greater than anterior circulation infarction group,among them the increase of24 hSBPCV, night SBPCV, night DBPSD and abnormal circadian rhythm 、 non dipper 、suggesting that the poor prognosis at 90 days.
