Abstract/Summary:
Aim: to study the characteristics of Blood Pressure Variability (BPV). Method: ambulatory blood pressure variability during 24h;daytime and nighttime in blood pressure (BP) 30 normal cases , 30 patients with hypertension at class one, and 25 patients with dilated cardiomyopathy(DCM).Results: (1)In DCM patients the 24h BP curve was relatively flattened, with the highest peak? at about noon . In daytime the BP level was?low and there was no remarkable BP decrease during the nighttime. The 24h systolic BP and daytime systolic BP as well as the 24h systolic blood pressure variability (SBPV) were significantly lower compared to the two other groups. (2)In DCM patients the decrease of BPV was not related to the degree of heart failure and practically not to the duration of the disease. The patients with severe heart failure and long DCM history patients had higher average BP. (3) Both the 24h and daytime SBPV and diastolic blood pressure variability (DBPV) were found to have a positive correlation with the degree of heart dilatation. The cases with highly dilated heart had even higher average BP. (4)The 24hDBPV,daytime DBPV and nighttime SBPV in female with DCM were found to he higher -4- than those of male; t~e result suggests that there is a relationship betweer BPV and sex. In DCM male patients the average diastolic B?was higher than females? Conclusion: In DCM patients the decrease of BPV is related to the disease itself. lack of activity~ specific pathological and pathophysiological changes. In DCM along with the difference in cardiac function .. degree of heart dilatation,, the duration of disease and sex, the average BP also had different manifestations. The significant decrease BPV in DCM patients can be used as a new reference index in determining its prognosis.
