Effect Of The Postoperative Analgesia After Open Surgery Of Colectomy Using Oxycodone Hydrochloride Injection In Different Time Point

Objective:To determine a better timing for oxycodone injection in surgery by comparing the influence of the postoperative analgesia after open surgery of colectomy using oxycodone hydrochloride injection in different time point.Method:We selected 60 patients undergoing colon cancer in schedule without significant difference. The patients match the following parameters: aging45 to 65 years old, ASA I- II, weighing 50-80 kg, they were divided into three groups randomly, each group includes 20 cases. After the routine anesthesia, Group A : injected with 10 ml physiological saline10 minutes before the skin-cut; Group B :injected with oxycodone 0.1 mg/kg 10 minutes before the skin-cut; Group C,injected with oxycodone 0.1 mg/kg10 minutes before the abdominal closed. All the groups connected and start the self-control vein analgesia pump which contained the same proportion of sufentanil and ramosetron. Recorded and compared the three groups of anesthesia time, operating time, extubation time(from stop drug to take out the endotracheal tube time) time of wake up(from stop drug to patients eyes open).Evaluated the scores at the time of 1 h(T1), 2 h(T2), 4 h(T3), 8 h(T4), 12 h(T5) after the surgery by using visual analogue scale(VAS) and comfort score(BCS). Record the effective press times of using PCIA in different period of time of threegroups. Also recorded the cases of nausea, vomiting,dizziness, respiratory depression situation.Results:(1)The general situation: There were no significant differences between the three groups when compared the anesthesia time, operating time, extubation time(from stop drug to take out the endotracheal tube time),time of wake up(from stop drug to patients eyes open)(P>0.05).(2) The pain scores: Comparison between groups: at each time in group B and C, scores were significantly lower(P<0.01) as compared with group A. At T1, T2, T3, T4 group B compared with group C grade reduced significantly(P<0.05).At T5, there were no significant differences between B and C group.(P >0.05).(3)The comfort score: The BCS average scores of group A at each time point scoring is below 2 points, and from T1 to T5,the scores tended to be increased. The scores at each time point of groups B and C were higher than group A. The average scores of the two groups were below 3,patients felt painful only when deep inspiration. The comparison between groups: at each time point in group B and C, scores were higher compared with group A(P <0.01). At T1, T3, T4, T5, each moment, there had no difference in B and C groups(P >0.05).(4) The times of effective press of PCIA: Comparison between groups: in every period of time, the times of group B and C were less than A group, the press time reduced significantly(P <0.05). The times of Group B were less than group C in every period of time(P <0.01).(5) Side effects: There were few side effects in the there groups,there was no statistically significant difference. No breathing inhibited in three groups.Conclusion:Injection with oxycodone 0.1 mg/kg before the skin-cut of the surgery can reduce the dosage of the PCIA, also can achieve a safe and satisfactory postoperative analgesia effect.
