Research Of Discharge Learning Needs And Influencing Factors Of Chronic Obstructive Pulmonary Disease Patients

Objectives:In order to investigate and understand the learning needs of patients with chronic obstructive pulmonary disease in Changchun,and to explore the related factors which can affect the learning needs.According to the analysis result,proposed targeted interventions to prevent and reduce the related factors affected their learning needs,and reduce the learning needs of the patients after discharge,promote self-regulation of the patients after discharge and rehabilitation,objective and scientific evaluation their learning demand conditions provide important reference for further improvement of hospital health education model to improve the effect of health education. Methods:We chose convenience sampling method from the three level of first-class hospital in Changchun,and we conducted questionnaire survey to 350 patients with chronic obstructive pulmonary disease who fit the requirements.Questionnaires include general population sociology data questionnaire,disease related questionnaire,Chinese edition of patient learning needs scale. 350 Questionnaires were distributed and 325 valid questionnaires were returned,the effective rate is 92.96%.We use Epidata3.1 software and the Excel2003 software to establish a database, and to double data entry,Then we import data into SPSS13.0 software and accomplish the statistical analysis.Data analysis include descriptive statistics,t test, ANOVA, Kruskal-Wallis H, pairwise comparison analysis of variance in both the number and multiple linear regression analysis. Result:1.The discharge learning need of COPD patients is high.The mean score of overall discharge learning need of COPD patients in Changchun is 132.03 ± 14.36 points(the percentage score after conversion is 66.02%).The scores five dimensions are:the mean score of knowledge of other diseases associated is 18.62±3.13 points(the percentage score after conversion is 67.02 %); the mean score of daily activities and treatment is 27.06±4.57 points(the percentage score after conversion is 67.65%);the mean score of symptom and complication is 29.20 ± 3.61 points(the percentage score after conversion is 73.00%);the mean score of medication knowledge is 26.69 ± 4.38 points(the percentage score after conversion is 66.73%);the mean score of social support and care is 30.42±4.74 points(the percentage score after conversion is 60.84%).2.The discharge learning need order of COPD patients from high to low is:symptom and complication 、 daily activities and treatment 、 medication knowledge,knowledge of other diseases associated,social support and care.3.ANOVA shows that the sociological demographic factors affecting the learning needs of COPD patients are age,marital status,education, occupation and payment methods.The differences of discharge learning need of COPD patients in Changchun five dimensions and overall discharge learning need in different age groups are statistically significance(P<0.05).The influence of different marital status on COPD patients is reflected in symptom and complication、medication knowledge and overall discharge learning need(P<0.05).Different level of education of COPD patients,its discharge learning need scores are significantly different.The difference of symptom and complication、medication knowledge and overall discharge learning need are statistically significance(P<0.05).The influence of different occupational groups on COPD patients is reflected in four dimensions and overall discharge learning need excepting other disease related knowledge(P<0.05).In according with the different payment methods,the differences of daily activities and treatment、symptom and complication、social support and care and overall discharge learning need(P <0.05).4.ANOVA show that the disease-related science factors affecting the learning needs of COPD patients are the number of hospitalizations, the course of the disease, the treatment and the type of medication.In according with the different number of hospitalizations for grouping,the differences of daily activities and treatment、symptom and complication、medication knowledge and overall discharge learning need(P<0.05).COPD patients who are different courses of the disease,the differences of daily activities and treatment,medication knowledge, social support and care and overall discharge learning need(P<0.05). Different treatments of COPD patients, its discharge learning need scores are significantly different.The difference of symptom and complication、daily activities and treatment and overall discharge learning need are statistically significance(P<0.05).The influence of different type of medication on COPD patients is reflected in symptom and complication, daily activities and treatment, medication knowledge and overall discharge learning need(P<0.05).5.Multiple regression analysis show that:The discharge learning need of COPD patients is influenced by occupation, education,the course of the disease,treatment and type of medication.High-cultural level in patients with lower cultural degree of patients discharged from hospital learning demand is high;professional for civil servants or teachers and farmers of COPD patients compared with professional for medical workers,the discharge learning need score is low;as the extension of course, the patient’s discharge demand gradually decline;drugs and long-term home oxygen therapy in patients with combined treatment of COPD patients compared with the simple drug treatment, the score is higher;combine traditional Chinese and western medicine treatment of the patients than just discharged patients treated with traditional Chinese medicine(TCM) learning demand is high. Conclusion:1.The discharge learning need of COPD patients is high.2.The discharge learning need order of COPD patients from high to low is:symptom and complication、daily activities and treatment、medication knowledge、knowledge of other diseases associated、social support and care.3.Age,marital status,education,occupation,payment methods,number of hospitalizations,course of the disease,treatment and type of medication are the factors influenced discharge learning need order of COPD patients,which occupation, education,the course of the disease,treatment and type of medication are the main factors.
