Background:Chronic obstructive pulmonary disease (COPD) is a worldwide common disease with high prevalence and mortality. It is one of chronic diseases which are mainly managed in our country. The prevalence rate of chronic obstructive pulmonary disease increases with age, so the physical and mental health of the elderly will be seriously affected. Global Initiative guidelines state that health education can improve the cognitive level of patients with chronic obstructive pulmonary disease, which is conducive to improving the quality of life, reducing anxiety, depression other symptoms. There were extensive researches on health education of patients with COPD which lay the foundation for nursing interventions. Health education should be based on an accurate assessment of the needs of patients, in order to identify problems and provide a more rational health education, the needs of patients should be fully understood. However most studies focused on health education methods and forms, there were few research on the need for elderly patients with COPD. Therefore, this study investigate the health education needs of elderly patients with COPD, in the hope to provide basis for scientific, systematic and personalized health education.Purpose:The purpose of this study was to describe and analysis the current status on knowledge level, health behavioral and health education needs of elderly patients with chronic obstructive pulmonary disease.Methods:Questionnaire was selected by literature review. A convenience sample of 258 elderly patients with chronic obstructive pulmonary disease who met the criteria of this research was recruited from Shanghai suburban community. The 258 elderly patients with chronic obstructive pulmonary disease were investigated by three questionnaires which were entitled by social demographic questionnaire, knowledge and behavior of elderly COPD questionnaire and health education need of elderly COPD questionnaire.Results:1. Knowledge in elderly patients with COPD:patients had better cognitive on the importance of diet and physical activity and risk factors such as smoking, infection, and air pollution. But with poor cognitive on the knowledge of oxygen, respiratory function exercise as well as medication.2. healthy behaviors of elderly patients with COPD:(1) respiratory function exercise: there were 42.6% elderly patients with COPD who have had done respiratory function exercises, which only 1.7% of elderly patients with COPD carried out every day, and 6.0% of elderly patients with COPD had frequent respiratory functional exercise. (2) Physical activity:58.3% of elderly patients with COPD had done physical exercise, but only 3.4% of patients insisted on physical exercise every day. (3) Diet and living habits:patients had good habits on diet and living,80.4 percent of elderly patients with COPD had regular life, and 77.4 percent of elderly patients with COPD with no picky about food.3. Health education needs of elderly patients with COPD:(1) mean total score was 8.57±3.49, needs of disease knowledge mean score was 1.40±1.05, needs of medication knowledge mean score was 1.30±1.01, the mean score was 2.50±1.23 on needs of self-management, mean score was 2.48±1.48 on the needs of physical activity knowledge and the mean score on needs of diet knowledge was 0.72±0.71. There are differences in different gender, educational level, marital status and mode of living on the health education needs.(2) There were 59.6% of patients want to know more knowledge about their medication; 32.3% of patients is not clear how much physical activity should be done and 56.6% of patients with COPD need to know how to eat.Conclusions:Elderly patients don’t have enough knowledge about COPD. There were differences between the needs of health education and knowledge about COPD in gender, educational level; physical activity respiratory function exercise conditions in elderly patients with COPD are poor; there is still room for improvement in elderly patients with COPD health education.
