Abstract:bjective:Comparison of ultrasound elastography Organization(EI) an color Doppler ultrasound(CDFI) in the diagnosis of breast mass quality,to explore the value of ultrasonic elastography in the diagnosis of breast lump.Methods:Through the summary analysis of 506 cases of breast mass patients, all of which are female patient, at the age of 21-79, average age was 43 years, including 382 cases of benign tumors (including breast fibroadenoma, breast cyst, breast disease with hyperplasia nodules, mastitis lump, lipoma),124 cases of malignant tumors (include medullary carcinoma, adenocarcinoma, invasive ductal carcinoma, non invasive ductal carcinoma etc), mass directly at 0.4-8.7cm. Color ultrasound diagnostic apparatus application of Hitachi EUB 7000,the instrumeng with color Doppler ultrasound and ultrasound elastography imaging function.Using high frequency prob,prob frequency in 5-10MHZ. the patients were performed ultrasound elastography imaging and color Doppler ultrasound examination, to observe the size, shape, internal echo mass, and the blood relationship with surrounding tissue, If the observed mass internal and peripheral blood flow signals increased, visible short a main vessel penetration, and that many branches, or see the abundant blood flow signal, namely the flow classification> II, and explore the high resistance of the artery, resistance index >0.75, blood flow velocity>25cm/s,, and even sometimes visible dynamic blood flow frequency spectrum of venous fistula, consider the masses tend to malignant, otherwise, tend to be benign;and ultrasound elasticity imaging observation organization, mass of color, If the block is blue said bag piece is hard,If the block is green said bag piece is soft, then for elastography, and measured the ratio of elastic strain rate, measured<3.08 mass is soft, measured >.3.08 mass is hard, through these comprehensive draws the preliminary judgment, and tracing the pathological examination results, summary analysis. At the end of the wrong diagnosis of ultrasound elastography imaging are analyzed, in order to improve the diagnostic rate. Results:In 506 patients, histologically benign operation in 382 cases, malignant 124 cases, ultrasound elastography imaging in the diagnosis of breast masses in malignant group, the diagnostic accuracy of 109, misdiagnosis of 15 people,363 benign group and accurate diagnosis, misdiagnosis 19, sensitivity 87.9% ultrasound elastography, a specificity of 95%, positive predictive the value 85.2%, and negative predictive value of 96%, color Doppler ultrasonography in diagnosis of breast masses in malignant group, the diagnostic accuracy of 97, misdiagnosis of 27 people,356 benign group and accurate diagnosis, misdiagnosis 26, sensitivity 78.2% color Doppler ultrasound examination, a specificity of 93.2%, positive predictive value of 78.9%, negative predictive value of 93%. In this paper,the pathological as standard, use fourfold table analysis of diagnostic test. The sensitivity,specificity,positibe predictive calculation of color Doppler ultrasonography and elastosonography value,negative predicitive value and the kappa statistic,calculation of the kappa value,P<0.05 had significant difference. Breast mass elastic hardness from large to small invasive ductal carcinoma>non invasive ductal carcinoma>breast fibrosis> mammary gland adipose tissue.Conclusion:Color Doppler ultrasonography has some deficiencies in the diagnosis of breast masses in nature. Ultrasound elastography than color Doppler ultrasonography can more accurate diagnosis of breast masses. Examination technology of ultrasonic detection technology, other image (such as molybdenum target X-ray, MRI) and ultrasound guided needle biopsy methods combination can improve diagnosis of breast tumors needs further study.
