Objective:Observing the relationship between efficacy of Pegylated Interferon (peg-IFN) therapy and Atg5 polymorphism and the constitution types of Traditional Chinese Medicine (TCM) in HBeAg positive patients with chronic hepatitis B (CHB).Method: ①Enrolled the cases about 120 patients with CHB of HBeAg positive who treated with peg-IFNα.②Divided those patients into yin deficiency group and non-Yin deficiency group according to Wang Qi’s classification for TCM constitution because of the limited sample.③Atg5 polymorphism was evaluated by polymerase chain reaction and restriction fragment length polymorphism(PCR-RFLP). ④Collected the conventional detection of liver function, hepatitis B three series, HBVDNA quantitative et al.⑤ Analysis of the relationship between efficacy of Pegylated Interferon (peg-IFN) therapy at 48 weeks and Atg5 polymorphism and the constitution types of TCM with CHB of HBeAg positive.Results:① The efficacy were different at 48 week treat by Peg-IFN a of the patients in HBeAg positive with CHB in the different groups of rs573775C/T polymorphism.The the rate of aminotransferases normalization, HBVDNA negative and HBeAg seroconversion were 63.3%、60.0%,13.3% in the TT group.The group of CC were 76.0%、92.0%、44.0%.And the group of CT were 64.6%、69.2%、21.5%. The efficacy of TT group was worse than others. It was not statistically significant in the rates of aminotransferases normalization (P=0.534).But the other rates were meaningful (P<0.05)②It is not in conformity with the Hardy-Weinburg balance law by the chi-square test of gene polymorphism distribution in the sample. The efficacy were different at 48 week treat by Peg-IFN a of the patients in the different groups of rs17587319C/G polymorphism。The rate of aminotransferases normalization, HBVDNA negative and HBeAg seroconversion were 48.4%,58.1%、9.7% in the GG group.The group of CC were 69.0%,70.7%,48.3%.And the group of CG were 80.6%,87.1%、38.7%. The efficacy of GG group was worse than others. These were statistically significant in all of the rates (P<0.05)③ The efficacy were different at 48 week treat by Peg-IFN a of the patients in the different groups of TCM constitution type. The the rate of aminotransferases normalization, HBVDNA negative and HBeAg seroconversion were 58.8%、60.8%、 13.7% in the group of yin deficiency,which were 72.5%,72.5%,31.9% in the group of non-Yin deficiency.lt was not statistically significant in the rates of aminotransferases normalization (P=0.117).But the other rates were meaningful (P<0.05)④Yin deficiency group showed the rate of CC,TT and CT in rs573775C/T were 44.0%,63.3%,32.3%.Which were 56.0%,36.7%,67.7% in non-Yin deficiency group. Yin deficiency group showed the rate of CC,TT and CT in rs17587319C/G were 34.5%,67.7%,32.3%,Which were 65.5%,32.3%,67.7%in non-Yin deficiency group. Thos were statistically significant in the rates of all (P<0.05)Conclusion:Both TCM constitution and rs573775C/T gene polymorphism affect the treatment response of peg-INFa for CHB of HBeAg positive, with the negative effects from yin deficiency and TT of rs573775C/T, but further study is required to clarify the association between them.
