To Research HLA-SNP Allelic Variation In Familial Aggregation Type Of Chronic Hepatitis B Patients With Deficiency Of Kidney-Yang Syndrome Based On The Theory Of Hidden Pathogen Due To Renal Deficiency And Its Prevention And Treatment

“ShenXuFuXie”,also known as “ShenXuXieFu”,is one of disease types which not attack immediately after human body getting pathogenic factors.The narrow sense refers to a special disease type due to deficiency of kidney Qi and lurking Shaoyin of seasonal febrile diseases caused by insidious pathogenic factors,broadly refers to all due to lack of kidney Qi caused by human body getting pathogenic factors and attack after a period of time.A lot of doctors think that “ShenXuXieFu” is the innate character of Chronic Hepatitis B(CHB)through theoretical and clinical research,they can treat Chronic Hepatitis B from the use of hidden pathogen theory.Chronic Hepatitis B family gather infection has the following characteristics:high incidence,high viral load,liver fibrosis,high incidence of liver cancer,liver damage,poor anti-viral effect.The study of HLA allele polymorphism is the focus of research on family aggregation and susceptibility to chronic HBV infection.It was found that HLA-DQB1*0201 and HLA-DRB1*0301 alleles may be associated with chronic hepatitis B with deficiency of kidney-YANG in Sichuan area.OBJECTIVE: To study the genetic susceptibility mechanism of chronic hepatitis B(HBV),to explore the relationship between HLA allele and Kidney-Yang deficiency syndrome in patients with Chronic Hepatitis B in Sichuan province.To provide evidence for elucidating the biological mechanism of kidney-Yang deficiency in chronic hepatitis B patients.To explore the prevention and treatment of chronic hepatitis B under the guidance of the theory“ShenXuFuXie”.Methods:42 patients with chronic hepatitis B family infection(Kidney-Yang deficiency syndrome,liver-qi deficiency syndrome,spleen-stomach damp-heat syndrome),chronic hepatitis B infection with non family aggregation(kidney-Yang deficiency syndrome)and 10 cases of healthy controls were collected.The genotypes of HLA-DRB1,DQB1,DPA1,DPB1 were detected by HLA-standard and sequencing method(PCR-SBT).Apply SPSS software to analyze the correlation among alleles and CHB kidney deficiency syndrome,family aggregation infection and laboratoryindex.Count data are tested through chi-square test.Measurement data results are represented by S,two sets of measurement data are tested through independent samples t test.Results: There were 14 HLA-DQB1 alleles,12 HLA-DPB1 alleles,19HLA-DRB1 alleles,5 HLA-DPA1 alleles in 52 samples.HLA-DPA1*0202-DPB1 *0501 were not statistically different between the groups,HLA-DPA1 * 0202-DPB1 *0301 were not detected.(1)HLA-DQB1 * 0303 and HLA-DQB1 * 0502,HLA-DRB1* 0901,HLA-DPA1 * 0103 may be associated with chronic hepatitis B in Sichuan.(2)HLA-DPA1 * 0103,HLA-DRB1 * 0301 and HLA-DQB1 * 0201 may be associated with family aggregation of Kidney-Yang deficiency syndrome of chronic hepatitis B in Sichuan.(3)HLA-DQB1*0201 and HLA-DRB1*0301 may be associated with Kidney-Yang deficiency syndrome of chronic hepatitis B infection in Sichuan.(4)HLA-DQB1,DRB1,DPB1,DPA1 alleles were not associated with viral load in patients with chronic hepatitis B in Sichuan.(5)There are some differences in the expression of HLA-DRB1*0406 in the ALT level of chronic hepatitis B patients in Sichuan area.Conclusion: HLA-DQB1*0201,DRB1*0301 are associated with the accumulation of kidney-yang deficiency and family aggregation.It reveals the genetic susceptibility of chronic hepatitis B and TCM kidney-Yang deficiency has certain relevance,TCM kidney-Yang deficiency with low immunity may lead to chronic HBV infection,familial kidney-Yang deficiency is likely to be a major cause of chronic infection of HBV genetic susceptibility,which initially interprets of the internal mechanism of chronic hepatitis B "Kidney-Yang deficiency-genetic susceptibility-“ShenXuXieFu”-family aggregation"Chinese medicine on the basis of syndrome differentiation and treatment,based on the “ShenXuXieFu” of the core pathogenesis,has remarkable effect to the treatment of chronic hepatitis B:improving liver function levels,improving the negative rate,inhibiting viral replication,breaking the immune tolerance effectively.In this study,we propose the treatment of chronic hepatitis B "bushenqingtou method" through the discussion of exploring traditional theory,combined withmodern research and clinical practice.Trying to play a positive role in treating chronic B Hepatitis carriers,family-infected patients,immune tolerance patients by strengthen kidney Qi,detoxification and eliminate pathogenic factors.
