ObjectiveGDM((Gestational Diabetes Mellitus) is one of the clinical common obstetrical diseases.It affects the health of not only pregnant women, but also the fetal. Macrosomia is one of the highest complications of GDM. RBP4 is a kind of specific proteins in the blood that is close with glucose metabolism. This research that is tested the serum level of RBP4 is to provide the basis of pathogenesis and the clinical prevention about GDM.MethodThe clinical datas were 146 prgnant women who were in the Women and Children’s Hospital of Linyi City from June 2011 to July 2014 that were all before selective cesarean section.GDM was disgnosed by the eighth edition of Obstetrics and Gynecology.46 cases of pregnant women were GDM group,100 cases of pregnant women without GDM were NGT group.After routine examinations,5ml elbow venous were all drew from them an hour before cesarean section,then they were separated by 1500 rpm for 10 minutes and the serum part was kept.RBP4 was tested by instruction and ELISA method,then the datas about RBP4, newborns weight,etc were recorded.Results16 macrosomias and 39 normal weight newborns were delivered in the GDM group,8 macrosomias and 92 normal weight newborns were delivered in the NGT group. The incidence of the macrosomia of GDM group was 34.8%,that of NGT group was 8.0%,it has statistical significance(P<0.05).The mean level of serum RBP4 of the GDM group was 27.775 mg/L,NGT group was 14.582mg/L,it has statistical significance(P<0.05). The mean level of serum RBP4 of the GDM group with macrosomia was 29.184mg/L, NGT group was 19.351mg/L,it has statistical significance (P<0.05). The mean level of serum RBP4 of the GDM group with normal weight newborns were 22.423 mg/L,NGT group was 14.167mg/L, it has statistical significance(P<0.05).There was a positive correlation between the RBP4 and FPG of all the pregnant women, B=3.258>0, OR=26.005>1 (P<0.05).In the NGT group, the mean level of serum RBP4 of the pregnant women with macrosomia was 19.351mg/L, normal weight newborns were 14.167mg/L, it has statistical significance(P<0.05).In the GDM group, the mean level of serum RBP4 of the pregnant women with macrosomia was 29.184mg/L, normal newborns weight was 22.423mg/L, it has statistical significance(P<0.05).Summarizing all the datas of the RBP4 and the weight of newborns to find the correlation,B=1.289> 0, OR=3.623>1,it has statistical significance(P<0.05).So we can see that the level of serum RBP4 is the risk factor of macrosomia and there was a positive correlation between them.Conclusion1. The macrosomia incidence of the GDM pregnant women was higher than that of the pregnant women without GDM.2. There is a positive correlation between RBP4 and GDM, the level of RBP4 of GDM are higher than the pregnant women without GDM.3. There is a positive correlation between RBP4 and the incidence of macrosomia, when level of RBP4 is high, the risk of macrosomia is high.
