Objective:To observe and study the expression and clinical significance of procalcitonin in acute purulent tonsillitis of children, as well as to evaluate comprehensively and objectively the curative effect about XuanShi pediatric self-established Xuanshi Qingjie Decoction by clinic observation when it is used on the basis of Western medicine to treat this disease, therefore providing scientific evidence for the popularization and application of this decoction in clinical. Methods:Using randomized, controlled, prospective clinical research method,60 cases that met the acute purulent tonsillitis were selected and then divided randomly into two groups: Antibiotic therapy group (which was infused the injectable Amoxycillin/Clavulanate Potassium) and Combined therapy group (which was treated with Xuanshi Qingjie Decoction and infused the injectable Amoxycillin/Clavulanate Potassium), while normal control group was established. Taking clinical symptoms, signs, laboratory indexes, ect. as the observation projects, we calculated the scale of decrease of primary symptom scores, secondary symptom scores and total scores, the main symptoms and signs improvement time, as well as the changes of laboratory indexes as criterion for effectiveness before and after treatment. SPSS 17.0 software was used for statistical analysis.Results:1. All groups before treatment were comparable in sex ratio, age distribution, duration of course, severity, and laboratory indexes (P>0.05).2. The primary symptom scores, secondary symptom scores and total scores in both two groups (Antibiotic therapy group and Combined therapy group) after treatment were declined markedly when compared with those before treatment (P<0.05), while the downward trend in Combined therapy group was more obvious (P<0.05).3.The time of fever clearance, sore throat relieving, tonsil atrophying and congestion mitigating in Combined therapy group were all shorter than those in Antibiotic therapy group (P<0.05).4.The expression level of PCT in both two groups (Antibiotic therapy group and Combined therapy group) were apparently higher than that in normal control group before treatment (P<0.05).5. The expression level of infectious indicators such as WBC, CRP and PCT in both two groups (Antibiotic therapy group and Combined therapy group) after treatment were declined markedly when compared with those before treatment (P< 0.05), the difference of the downward trend of WBC and CRP between two groups was not statistically significant (P>0.05), while the downward trend of PCT in Combined therapy group was more obvious (P< 0.05).6. The cure rate was 6.67%, the remarkably effective rate was 53.33%, the effective rate was 33.33%, the ineffective rate was 6.67%, and the total effective rate was 93.33% in Antibiotic therapy group, while the cure rate was 30.00%, the remarkably effective rate was 60.00%, the effective rate was 6.67%, the ineffective rate was 3.33%, and the total effective rate was 96.67% in Combined therapy group. The overall curative effect in Combined therapy group was superior to Antibiotic therapy group (P<0.05).Conclusion:1.PCT is a index with high sensitivity and high specificity for acute purulent tonsillitis of children, which helps early diagnosis and can effectively reflect the treatment effect of this disease, thereby play a guiding significance for subsequent treatment.2. The use of Xuanshi Qingjie Decoction on the basis of Western medicine can improve the clinical symptoms and physical signs of children with acute purulent tonsillitis more efficiently and more quickly, and can control the infectious level significantly, thus showing a good clinical curative effect, which is worth of promotion.
